Responsibilities
This is a PRN position
In this inspiring role you hold the power to perform comprehensive assessments collaborating seamlessly with our multi-disciplinary team of experts including PT OT ST MSW and paraprofessionals. Together we will work passionately to achieve the heartfelt outcomes and goals of our patients leading them on a journey of healing and hope.
Supervise your caseload nurturing the growth and development of LPNs and Home Health Aides as we cultivate a team of caregivers. In your role as a facilitator of care coordination we celebrate the unity that arises when PRN staff steps in ensuring seamless support for your patients. We welcome you to a role you can grow in!
Qualifications
- Active RN License
- One year RN nursing experience or three years LPN/LVN nursing experience providing direct patient care in a home care setting.
- Current CPR Certification
- Valid Drivers License
