Are you an experienced registered nurse physiotherapist occupational therapist social worker dietitian or speech language pathologist seeking a rewarding career that cares for others in a professional practice that cares for you Youre looking in the right place.

As a Psychogeriatric Resource Consultant you will provide consultation and case-based education to staff caring for clients with complex physical and mental health needs in Ontario Health atHome community agencies long-term care facilities and acute care settings. In collaboration with St. Josephs Health Centre Guelph and the regional BSO Lead plans and delivers capacity-building training and BSO education programs such as the 5 STaR Programs PIECES UFIRST and BSO Foundations

What will you do

  • Provides consultation and education to staff that care for clients with complex physical mental health needs with associated behaviour in Ontario Health atHome community agencies LTC facilities and acute care sites.
  • Plans and delivers education/training sessions for capacity building and provides case-based consultation to assist with assessment care and treatment planning.
  • Collaboration with St. Josephs Health Centre Guelph regional BSO Lead to deliver BSO education (Behavioural Supports 5 STaR Programs PIECES UFIRST BSO Foundations) and programing to acute and long-term care sectors
  • Performing other duties as assigned.

What must you have

  • Knowledge and application of case management principles and practices acquired through membership in good standing with the appropriate college as a Registered Nurse Occupational Therapist Physiotherapist or Speech Language Pathologist Registered Dietician or membership in good standing with the College of Social Work with a degree at the Masters level preferred.

PRCs provide clinical consultation to professional caregivers by:

  • Facilitating knowledge to practice by empowering teams to engage in problem solving and solution finding when managing Responsive Behaviours using the P.I.E.C.E.S. framework
  • Assisting staff in the application and interpretation of screening/assessment tools e.g. BSO-DOS CMAI Cornell GDS and relevant InterRAI Outcome Scores such as ABS CPS and DRS
  • Supporting person-centred practices among interdisciplinary providers on a case-by-case basis by guiding the development of specialized behavioural care plans and targeted interventions.
  • Assist staff with risk screening for complex clients/residents and facilitating crisis interventions as needed e.g. acute care and schedule 1 facilities
  • Promoting system navigation facilitating linkages and supporting partnerships between hospitals LTC and community services (including but not limited to OH aH CC BSO CRBT BSO TT SGS Inpatient programs Community Support Agencies Adult Day Programs Primary care Hospital GEMs) for individuals/families to enhance quality care and promote smooth transitions.
  • Facilitating as required complex care resolution in a manner that promotes team collaboration and an integrated and client-centred approach to care
  • Support the implementation of evidence based tools and best practices across the continuum of care (e.g. Canadian Coalition of Seniors Mental Health National Guidelines and the Health Quality Ontario Standards)
  • Specialized Certification (e.g. Canadian Nurses Association Certification in Gerontology or Psychiatric/Mental Health Geriatric Certificate Program through McMaster University Health Sciences) an asset
  • 3-5 years of progressive experience and developed expertise in mental health and behavioural health assessment consultation intervention strategies and care planning for the defined population is required
  • Demonstrated knowledge and understanding of the most common dementias other neurological disorders delirium mental illness and addiction issues experienced by the defined population
  • Demonstrated expert understanding of Responsive Behaviours i.e. how to define and understand associated risks including use of standardized assessment tools how to explore triggers and develop effective intervention strategies (both non-pharmacological and pharmacological) with staff
  • Knowledge and demonstrated experience in working within the P.I.E.C.E.S. framework certified as a P.I.E.C.E.S. Educator preferred
  • Knowledge and demonstrated experience with GPA certified GPA Coach preferred
  • Knowledge and demonstrated experience with U-FIRST certified U-FIRST Trainer preferred
  • Experience in identifying learning and development needs and providing education to a variety of sectors including the application of adult learning principles; completion of Dialogue Education methodology an asset
  • Experience in developing and delivering case-based clinical education and to support staff in the application of new knowledge to practice
  • Demonstrated leadership facilitation coaching consulting and mentoring skills; Management experience is an asset
  • Demonstrated knowledge and basic understanding of the
  • Fixing Long Term Care Act 2021
  • Bill 168 Ontarios Law on Workplace Violence and Harassment
  • Mental Health Act
  • Health Care Consent Act
  • Substitute Decisions Act including understanding the role/scope of the PG&T
  • Personal Health Information Protection Act (PHIPA)
  • Excellent understanding of best care practices for the target population
  • Well-developed knowledge of the local service system for this population including crisis services Seniors at Risk and vulnerable older adult resources
  • Proficiency in the operation of a personal computer
  • Ability to set and meet deadlines and work independently
  • The ability to travel throughout Waterloo Region and Wellington County is required.
  • Fluency in French language preferred.

  • Ability to speak French or another second language
