Position Summary:
Under the supervision of the the Transitional Care Center (TCC) operational and clinical directors the RN Navigator serves as a coordinator of clinical care for patients requiring assistance and support in accessing and managing their health care needs. The RN Navigator connects with the patient and caregiver at regular intervals driven by patient acuity to assess for overall wellness symptom burden identify and resource for barriers to care provide emotional support and patient education. The TCC RN collaborates with the supervising provider and serves as a liaison between providers/care settings to ensure the delivery of competent compassionate and age appropriate patient care to prevent unnecessary ED visits acute admissions or other untoward outcomes.#
