Job Description

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. In this critical role you will provide essential nursing care to patients in various settings including hospitals clinics and long-term care facilities. As an LPN you will work under the supervision of registered nurses and physicians delivering high-quality care that meets the physical mental and emotional needs of our patients. Your responsibilities will encompass a wide range of clinical tasks from taking vital signs and administering medications to assisting in patient assessments and developing care plans. You will also have the opportunity to build meaningful relationships with patients and their families ensuring they receive the support and information necessary for optimal health outcomes. Responsibilities

  • Provide direct patient care including monitoring vital signs and assessing patient conditions.
  • Administer medications and treatments as prescribed by registered nurses or physicians.
  • Assist in the development and updating of patient care plans based on individual needs.
  • Maintain accurate patient records and documentation to ensure continuity of care.
  • Support patients and their families with emotional and educational support regarding health conditions.
  • Collaborate with healthcare team members to optimize patient outcomes and discharge planning.
  • Ensure a clean and safe environment adhering to infection control practices and safety standards.

Requirements

  • Graduate of an accredited school of professional nursing.
  • Current license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse in the state(s) of employment.
  • Minimum of one (1) year experience in an acute care setting or equivalent experience.
  • CPR certified
  • Demonstrated written verbal and interpersonal communication skills.
  • Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Has a good driving record.
