What Youll Do:
The Physical Therapist (PT) will provide PT based services to patients in the comfort of their homes.
Assess and evaluate patients and develop plans of care (POC).
Provide treatments as required in accordance with established POCs.
Provide thorough and prompt documentation of progress during episodes.
Adhere to Brightstar Care standards for patient care safety and communications.
Be willing to travel within an established radius.
Assure compliance with all local state and federal laws and regulations

Qualifications:
- Degree from an accredited Physical Therapy institution.
- Licensed Physical Therapist (PT) in Texas.
- At least one year of experience in a home health adult and pediatrics experience preferred.
- EMR/EHR (KanTime) experience preferred but not required will train.
- Valid Drivers License and Car Insurance.

Job Type: PRN
Pay:
- $115.00 per visit/hour for OASIS (Medicare) SOC/IA and Recerts.
- $95.00 per visit/hour for Non-OASIS SOC/IA and Recerts.
- $65.00 per visit/hour for Treatments and Agency DC.
