Occupational Therapist
Part Time: 0.6 FTE
Day Shift:
- 8:00 AM - 5:00 PM
- 3 Days/Week
- Weekend On-Call Rotation
- 2 Holidays/Year

Responsibilities
- Deliver In-Home Occupational Therapy - Provide skilled occupational therapy services in patients homes including assessments treatment planning and interventions aligned with physician orders and individualized care plans.
- Coordinate Care and Ensure Quality Outcomes - Collaborate with interdisciplinary team members patients and caregivers to develop implement and update care plans that promote patient independence prevent hospitalizations and achieve measurable health outcomes.
- Maintain Clinical Documentation and Compliance - Ensure accurate timely documentation and compliance with regulatory accreditation and agency standards including participation in quality improvement training and performance initiatives.
Qualifications
Education:
- Graduate of an occupational Therapist education program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association Inc. (AOTA) or successor organizations of ACOTE
- Eligible to take or has successfully completed the entry-level certification examination for Occupational Therapists developed and administered by the National Board for Certification in Occupational Therapy Inc. (NBCOT)
Experience:
- One year experience caring for similar patient population as to be assigned.
License(s)/Certification(s):
- Licensed or otherwise regulated if applicable as an Occupational Therapist by the state in which practicing unless licensure does not apply in the practicing state.
- Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
- Mandatory Reporter: Child & Dependent Adult Abuse.
- CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
