Job Description
Hours: full-time (37.5 hours per week) Monday-Friday 9 AM-5 PM. Schedule dependent on provider and client needs.
Location: Lynchburg VA 24501
Position Summary
The Licensed Practical Nurse (LPN) working in the Horizon Wellness Clinics are responsible for the overall medical care of assigned individuals including the service delivery of basic nursing assessments; delivery of client/family education and diagnostic procedures; consultation with physicians; and carrying out of doctors orders.
Required Education and Experience
- The successful completion of a course of study that allows for licensure as a Licensed Practical Nurse in the state of VA is required.
- One year of experience providing nursing services as an LPN is required or an equivalent combination of education and experience that provides the required knowledge skills and abilities.
Related Knowledge Skills and Abilities
- Knowledge of community mental health and public health in order to provide advocacy and enhance an individuals treatment and access to community resources; Knowledge of nursing best practices.
- Knowledge of the principles procedures techniques trends and literature related to mental health physical health intellectual disabilities and substance abuse.
- Willing and available to work modified schedules (weekends holidays etc.) as required.
Ability to drive and must possess a valid drivers license. May be called upon to travel to any Horizon worksite; transport clients provide services to clients at other locations. Regular attendance at trainings and Horizon meetings may require travel to a variety of Horizon sites.
