Application Deadline:June 2 2025 by 11:59 pmProgram Overview Counselling & Support Services This position is responsible to the Manger of Housing Access Partnerships and Tenant Supports for the provision of enhanced housing help and relocation initiatives. Operating within the Housing and Housing Access Department this position supports the Centres primary goal of helping individuals in the community and those living on low income to find and keep appropriate housing and prevent evictions wherever possible. The philosophy of this work is in keeping with WoodGreens commitment to: participate in the building of sustainable communities social justice and creating environments free of discrimination or any other barriers which impede the full development of services usersWhat You Will DoHousing Support and Relocation Activities
  1. Provide information referral services and ongoing support to users needing assistance locating housing.
  2. Conduct a comprehensive intake and assessment of clients income background and service needs to identify the most suitable housing options. This includes evaluating income levels access to services and other specific requirements to ensure tailored and effective housing solutions.
  3. Provide direct support to clients navigating the relocation process ensuring smooth transitions into stable housing.
  4. Collaborate with community agencies to coordinate services such as transportation moving logistics and essential needs setup.
  5. Act as a liaison to address client concerns advocate for resources and promote successful housing outcomes while fostering strong community connections.
  6. Liaise with community resources on behalf of users where necessary.
  7. Keep month statistics and complete reports on service activities.
  8. Take part in maintaining the comprehensive collection and updating of resource materials.
  9. Provide services and strategies to stabilize housing for people at risk of homelessness including landlord mediation and support with navigating the Landlord and Tenant Board (LTB).

Housing Access Activities
  1. Provide support to clients families and community partners in accessing and processing WoodGreen Community Housing applications for independent and support units.
  2. Coordinate intake with applicants and community partners to ensure completeness of applications and supporting documentation required for WCH application.
  3. Determining the eligibility of referrals confirming housing to eligible applicants notifying ineligible applicants and providing information around alternative programs
  4. Maintain ongoing communication with external housing providers/partners most notably with front line staff to confirm applications and intake processes are completed including providing feedback to applicants
  5. Take part in team and unit meetings; and participate in regular supervisory sessions with the Housing Access Partnerships and Tenant Support Manager.
  6. Undertake the entering of new applications and documenting access applications in the system of record
  7. Assist in the creation and development of necessary policies procedures & guidelines as needed.
  8. Perform other related duties as required by the Unit Director of Housing and Manager of Housing Access Unit.
Training & Development activities:
  1. Familiarize with policies documents and strategizes with regards to emerging needs and issues in the community for those looking to access independent social housing
  2. Participant to community forums/town hall meetings on housing issues.
  3. Plan and implement ongoing promotion/awareness building of WCH and its services to the immediate and broader community.
  4. Familiarize yourself with WoodGreen partnerships and requirements for housing with supports and Affordable housing partners
  5. Participate in internal team building activities and connect with key stakeholders to ensure an integrated customer service focused work culture
  6. Ensure the antiracism objectives and community development principles of the organization are achieved within the program.
What You Bring to the Team
  • Social service related degree or equivalent
  • Two (2) years direct experience in providing housing help and/or information and referral services.
  • Strong intake and assessment skills; advocacy; negotiating skills.
  • Demonstrated information and referral skills.
  • Experience working with homeless/street involved individuals psychiatric survivors tenants and landlords.
  • Experience in workshop development community organizing and community development.

  • Knowledge of resources for vulnerable low-income singles in the City of Toronto
  • Knowledge of local resources in East Toronto
  • Previous experience with housing relocation services and referral support.
