Job Description

SUMMARY

Provides frequent resident/client assessments monitoring of residents health care status medication administration and related challenges. Consults with physicians dentists therapists pharmacists and hospitals to ensure high quality health care to Key Training Center residents. Performs nursing responsibilities in accordance with Florida professional standards licensure and competency qualifications.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  1. Assesses and treats resident injuries as needed.
  2. Assesses residents daily at various group homes as well as any other medically fragile residents assigned by the Nursing Supervisor. Reports findings to Nursing Supervisor.
  3. Documents assessments as need for residents.
  4. Monitors quantity of medications for compliance with APD/AHCA group homes.
  5. Trains staff and documents same on in-service form(s). Forwards originals to Nursing Manager and HR.
  6. Ensures all validations and training documentation for staff is available and kept in an organized manner.
  7. Conducts weekly med reviews for residents at group homes; forwards documentation to Nursing Supervisor.
  8. Reviews incident reports seizure reports and medical reports.
  9. Collaborates with Resident Manager and Residential Services Supervisors to discuss medical issues.
  10. Collaborates with Resident Manager Residential Services Supervisor and Nursing Manager when a medical emergency arises or regarding complicated medical conditions.
  11. Monitors residents at meal times to ensure diet orders and portion sizes are appropriate. Repots findings to RM and RSS.
  12. Administers medication as needed.
  13. Contacts physicians to clarify orders obtain new prescriptions request adjustments in medication schedule and other medical issues that may arise.
  14. Assists with scheduling appointments; ensures medical appointments are kept. Accompanies staff on appointments for medically fragile residents.
  15. Identifies safety/health/hazards; gives recommendations for corrections(s) to RM.
  16. Ensures maintenance of adaptive equipment and medical supplies.
  17. Reviews/maintains support plans for residents if needed.
  18. Attends monthly staff meetings.
  19. Performs all other essential duties or assignments as required or requested by the Residential Services Manager Director of Housing & Residential Services Nursing Manager or the Director of Programs and Services.

QUALIFICATIONS To perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge skill and/or ability required. EDUCATION and/or EXPERIENCE

Licensed LPN in the State of Florida. Minimum of one year nursing experience. Experience with developmentally disabled adults preferred.

CERTIFICATES LICENSES REGISTRATIONS

Florida State Board of Nursing - RN/LPN CPR/First Aid certified. Must be trained in HIV/Aids and infection control.

OTHER SKILLS AND ABILITIES

Ability to deal courteously tactfully and efficiently with resident families staff members and the general public.

Emotional stability and maturity with a genuine interest in working with residents.

Capable of working a flexible hour/day or weekend schedule.

Knowledge of life-safety and infection control protocols.

Willingness to comply with in-service training or other mandates.

General knowledge of behavioral management practices.

Ability to maintain confidentiality.

Ability to recognize and assess unsafe/hazardous conditions.

Knowledge of the provision of the American with Disabilities Act (ADA) particularly as it relates to a barrier free environment.

While performing the duties of this job the employee is regularly required to talk or hear. The employee is frequently required to stand; walk; sit; use hands to finger handle or feel objects tools or controls; and reach with hands and arms. The employee must frequently lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision distance vision color vision peripheral vision depth perception and the ability to adjust focus.

While performing the duties of this job the employee frequently works in outside weather conditions and is occasionally exposed to toxic or caustic chemicals. The noise level in the work environment is usually moderate.

OCCUPATIONAL RISKS:

This position involves intermittent exposure to blood body fluids or tissue. Protective measures are available. Hepatitis B Vaccine is recommended.

There is a potential for injury in this position if proper safety procedures and protocols are not followed or if violations are not promptly reported

Errors in judgment and decisions can result in negative outcomes for residents employees families and the facility. Deviation from established professional practices/standards could result in serious consequences for the organization the residents it serves and credibility of the facility (residence). Deviation from fire and life-safety codes could result in serious consequences for residents employees families and visitors.
