JOB SUMMARY
The System Care Coordinator (SCC) works in collaboration and partnership with colleagues patients and their caregivers as well as clinic hospital and community partners to achieve project goals and objectives. Using a defined process the SCC identifies Social Determinates of Health needs of pregnant and postpartum women creating a patient centered individualized plan for navigation into partner programs scheduled follow-up contact to reassess need and collect qualitative data (client stories) and routing women through the care system for up to one-year post-pregnancy.The SCC will manage and track referrals by leveraging an electronic closed loop referral system. Additionally the SCC will compile analyze and report site specific data which will be used to identify trends improve workflow and contribute to quality improvement conversations.
The SCC works with the team to ensure successful replication of Clinical-Community Integrated Care Coordination model which may include sharing project outcomes and attending educational opportunities.
JOB DETAILS AND REQUIREMENTS
Type: Full Time (80 hours per 2 week pay period with benefits)
Typical Hours: Day Shift Monday - Friday
Education:
-Graduate of an accredited school of nursing- required
-Bachelors of Science in Nursing degree- preferred
Certification/Licensure:
- Currently licensed to practice as a Registered Nurse in the State of Missouri- required
-BLS Certification- required
- Valid drivers license and ability to travel to various locations throughout the Missouri Bootheel- required
Experience:
- At least two (2) years of nursing experience- required
- Ability to work independently and exercise sound judgment in interactions with physicians colleagues patient members and their families
- Excellent interpersonal communication and negotiation skills as well as effective oral and written communication
- Demonstrated leadership skills
- Strong analytical data management and computer skills
- Strong organizational and time management skills as evidenced by capacity to prioritize multiple tasks and role components
- Experience working in interdisciplinary teams
Additional Requirements:
- Nursing perinatal health education and outreach preferred
- Experience in public health and cultural competency preferred
- Community resource knowledge preferred
- Working knowledge of the Missouri Bootheel and its socio-economic issues- preferred
- Must be able to set up a home office and meet the SFMC telecommuting expectations for remote working
- Must have access to reliable internet
