Location:The Lung Health Foundation is based in Toronto and operates in a hybrid model in which staff are required to work at the office a minimum of two days each week with occasional travel within Ontario and across Canada as required.

POSITION OVERVIEW
The Director of Programs & Patient-Centered Care is a key strategic leader at the Lung Health Foundation (LHF) responsible for shaping and scaling high-impact evidence-based programs that improve the health outcomes and lived experiences of individuals affected by lung disease. This role is central to advancing LHFs mission by ensuring that all programs are grounded in the best available evidence responsive to the needs of diverse communities and aligned with the Foundations strategic priorities.
Reporting to the Vice President Programs and Health Systems Partnerships the Director will lead the design integration and execution of patient-centered initiatives across multiple priority areas including asthma chronic obstructive pulmonary disease (COPD) and lung cancer. The Director will drive innovation in care delivery by leveraging data implementation science and health system knowledge to enhance access quality and patient empowerment.
This is a high-impact leadership role that requires a combination of strategic vision clinical and public health insight operational excellence and a deep values-driven commitment to advancing health equity and improving outcomes. The Director will work cross-functionally with internal teams and external partnersincluding clinicians policymakers researchers and patient communitiesto build and sustain programs that are both person-centered and system-informed.
KEY RESPONSIBILITIES
People Leadership & Team Culture
- Manage a cross-functional team fostering a culture of collaboration excellence and impact.
- Lead project planning budgeting risk management and performance tracking across patient-focused initiatives.
- Ensure compliance with funder requirements and contribute to grant writing reporting and sustainability planning.
- Provide inspiring and inclusive leadership that motivates team members and aligns them with organizational goals.
- Foster a culture of collaboration psychological safety and continuous learning.
- Mentor and support team members in their professional development through regular feedback and growth opportunities.
- Promote clear communication accountability and shared ownership of results.

Program Leadership & Strategy
- Lead the design delivery and evaluation of patient-centered programs ensuring alignment with evidence-based practices and the lived experiences of those affected by lung disease.
- Drive innovation in program delivery to better support self-management care navigation and patient empowerment.
- Identify gaps and opportunities in care access equity and qualityand design initiatives to address them.
- Champion the integration of Certified Respiratory Educators (CREs) and other supports into primary care and community-based models.
Stakeholder Engagement & Advocacy
- Build and maintain strong relationships with patients caregivers healthcare providers researchers policymakers and system partners.
- Lead collaborative initiatives that connect patients to care and amplify their voices in policy and system change.
- Represent LHF in key forums and advisory roles to elevate patient needs and influence health system planning.
Health Equity and Inclusion
- Championing a health equity lens across all programs ensuring initiatives address the diverse needs of communities disproportionately affected by lung disease.
- Leading the development of inclusive accessible materials and patient-centered care models.
- Embedding equity considerations in organizational strategy evaluation frameworks and policy recommendations.
- Fostering a culture of inclusion psychological safety and respect across internal teams and external partnerships.
- Demonstrating fluency or familiarity with anti-oppression frameworks intersectionality and the social determinants of healthand integrating these concepts into practice.
Evidence-Based Assessment & Insights
- Overseeing the collection interpretation and strategic use of quantitative and qualitative data to guide decision-making and program improvement.
- Leading or commissioning literature reviews environmental scans and best practice research to inform program and policy development.
- Identifying system-level trends population health gaps and emerging issues in lung health to shape strategic direction.
- Developing and presenting evidence-informed recommendations to senior leadership partners and funders.
- Embedding a culture of measurement reflection and continuous learning within the team and across initiatives.
QUALIFICATIONS
- A graduate degree in public health health administration health sciences social work or a related field (equivalent experience may also be considered).
- Minimum of 7 years of progressive leadership experience in healthcare public health community health or non-profit sectors.
- Proven track record in developing and leading patient-centered programs with demonstrated success in improving health outcomes and service equity.
- Experience managing cross-functional teams and collaborating with diverse stakeholders including healthcare professionals community organizations and policymakers.
- Knowledge of lung health issues including asthma COPD lung cancer tobacco cessation or chronic disease management is a strong asset.
