Job Description

Job Summary and Qualifications:

  • Summary of Duties

The Patient Care Manager (PCM) will be responsible for leading and managing the CVICU and RAPID Teams. The PCM will facilitate and champion problem solving and decision making within and across teams and is accountable to monitor and evaluate results of key performance indicators of quality and recommend ways to improve care and facilitate achievement of strategic and operational objectives. The PCM is a values-driven leader with demonstrated ability to build high performing interprofessional teams who excel in creating positive patient and family experiences.

The PCM will be primarily responsible for ensuring the provision of patient care consistent with the goals objectives and standards of the program. The PCM will co-ordinate day-to-day utilization of human and material resources to achieve program goals and functions; monitor attendance and performance of team members in achieving standards of practice and manage performance reviews; and achieve financial targets by monitoring unit-based expenses. The successful incumbent is also accountable for ensuring patient care requirements are met with appropriate competence and delivery and that patient care issues problems and concerns are resolved. The PCM works in collaboration with critical care and hospital leaders and medical staff to ensure to co-create an environment consistent with our hospitals mission vision and values.

The successful incumbent reports to the Director of Operations Tory Trauma Program. The PCM has oversight of expenses and revenues within a multi-million dollar budget and will be responsible for supporting a large team of professionals and unregulated health care workers. The PCM works in collaboration with Tory Trauma Program leaders and other hospital leaders (Professional Practice Leaders Advanced Nursing Practice Leaders and Clinical Educators) and physicians to advance strategic and operational goals at the unit program and hospital level.

Qualifications/Skills

    • Current registration in a professional regulatory college
    • Masters degree completed
    • Minimum of 5 years experience in Critical Care or related clinical experience is mandatory
    • Minimum of 3 years recent leadership experience in an acute setting preferably in a manager role is preferred
    • Proven organizational interpersonal and communication skills
    • Proficiency in working with computers and systems (i.e. MS Office Outlook virtual meetings etc.)
    • Systems knowledge including Critical Care Services Ontario (CCSO) Critical Care Information System (CCIS) Trillium Gift of Life (TGLN) and others
    • Demonstrated commitment to patient and family experience equity and social accountability
    • Proven interpersonal skills and a relational and values-based approach to coaching and management
    • Proven leadership in fostering a respectful work environment team building conflict resolution and negotiation
    • Demonstrated commitment to academic excellence and innovation
    • Demonstrated success in establishing leading and maintaining positive change and continuous quality improvement
    • Demonstrated ability to plan organize set and accomplish goals and objectives in a complex fast paced environment
    • Demonstrated ability to work in an interprofessional team and collaborate with others in the delivery of care to patients
    • Ability to work effectively and with composure under pressure/stressful conditions and meet multiple and competing deadlines
    • Working knowledge of applicable legislation and regulations impacting the management of human resources (i.e. Human Rights Employment Standards Occupational Health & Safety Act Unions)
