Job Description
THE POSITION

DESCRIPTION OF WORK
In this position you will provide care management services to older adults in need of protective services by developing care plans identifying resources and assessing the level of care needed to effectively support each person. Your work will involve obtaining information to identify social economic emotional health or physical needs of our consumers and assisting them in obtaining a variety of services. This includes developing implementing monitoring and modifying person centered care plans to support older adults as well as working in close cooperation with other social agencies hospitals clinics courts and community resources in planning to meet the needs of consumers and assisting the consumer in utilizing resources. Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:00 AM to 4:00 PM Monday - Friday with a 60-minute unpaid lunch.
- Telework:You will not have the option to telework in this position.

REQUIRED EXPERIENCE TRAINING & ELIGIBILITY
QUALIFICATIONS
Minimum Experience and Training Requirements:
- Six months as an Aging Care Manager 1 or a County Caseworker 1; or
- Successful completion of the County Social Casework Intern program; or
- Six months of professional human services or nursing experience; and a bachelors degree in social sciences behavioral sciences human services or a closely related field; or
- An equivalent combination of experience and training.

- Hiring preference for this vacancy may be given to candidates who live within Perry County. If no eligible candidates who live within Perry County apply for this position candidates who reside in other counties may be considered.
- You must be able to perform essential job functions.
Legal Requirements:
- This position falls under the provisions of the Older Adult Protective Services Act.
- Under the Act a conditional offer of employment will require submission and approval of satisfactory criminal history reports including but not limited to PA State Police and FBI clearance.
