Senior Clinical Manager Position Summary: Responsible for the oversight and supervision of the clinical homecare staff. Assures patient care is delivered in compliance with Compass Regional Hospice (Compass) policy and procedures as well as state and federal regulations and industry standards. The Senior Clinical Manager is a member of the Senior Leadership Team (SLT). Reports to: CEO Supervisory Responsibilities: This position is responsible for the direct supervision of the RNs Homecare CNA Manager and the overall function of the home care program. Responsibilities & Functions: - Coordination of direct and indirect patient services provided by all clinical staff.
- Provides guidance and counseling to all clinical staff to assist them in improving all aspects of hospice care services.
- Assists clinicians in setting priorities in developing and establishing immediate and long-term therapeutic plans of care.
- Assists the administration in the preparation and administration of the annual budget.
- Interviews hires and terminates clinical home care staff.
- Identifies need for clinical staff education and assists in planning and implementing yearly in-service and continuing education programs.
- Maintains and delegates the maintenance of electronic clinical records statistics reports and records for purposes of evaluation and reporting of organization activities. Assures proper maintenance of medical records as prescribed and in compliance with local state and federal laws.
- Maintains or delegates the maintenance of adequate and appropriate inventory supplies for the provision of patient services.
- Promotes hospice care in the health care community. Maintains good standing in the medical community.
- Conducts patient visits to offer support and expertise to the nursing staff patients and families.
- Serves as a resource ensuring that patient problems are addressed and resolved in a timely manner.
- Ensures that staff understands and exhibits the hospice philosophy of palliative care through education and role modeling.
- Provides daily oversight to ensure that homecare admissions are processed in an efficient and effective manner ensuring timely patient response and services.
- Ensures that all referral and admissions activities and processes are functioning in a manner that will optimize growth while in compliance with all clinical operation and regulatory requirements.
- Completes and delivers performance evaluations in an objective and timely manner.
- Interviews hires coordinates training and manages all homecare clinical staff.
- Participates in clinical liaison visits.
- Participates in and supports continuous process improvements.
- Works in conjunction with the hospice medical directors and hospice CRNP in the provision of effective symptom management of all patients.
- Is responsible for the nurse on-call schedule.
- Provides education to patients and families specific to the hospice benefit end of life care and symptom management.
- Initiates communication with attending physicians other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
- Confers with appropriate members of Compass staff to ensure patient changes are managed in a timely manner ex: changes in patient level of care contemplated discharges and patient hospitalizations.
- Attends IDG regularly.
- Meet regularly with hospice nursing staff to review problems or unique issues from caseloads share professional support and exchange feedback aimed toward enhancing professional growth.
- Takes responsibility and accountability for own nursing practice continuing education and professional development.
- Offers oversight to medication management in the homecare setting to ensure medications and medication costs are managed efficiently by homecare case managers.
- Offers oversight to the homecare clinical staff to ensure patient documentation reflects appropriate diagnosis codes and up to date information in accordance with legal and regulatory standards.
- Approval of clinical bills relating to homecare patients keeping the best interest of patients in mind while being financially responsible on behalf of Compass.
- Accountable for meeting patient care needs 24/7 via appropriate scheduling and delegation to members of the clinical team.
- May function as an admitting nurse or case manager when needed.
- Participate in agency and community programs as requested to promote professional growth and understanding of hospice care.
- Demonstrate familiarity with policy and procedures of the agency and rules and regulations of State and Federal bodies which aid in determining policy & procedures.
- Maintains the confidentiality of employee patient/client and agency information at all times.
- Acts as a resource for billing/finance staff in regard to clinical operations.
- Other duties as assigned by the CEO.
Qualifications: - RN degree in nursing from an accredited college or university
- Possession of a current license to practice as a registered professional nurse in the State of Maryland.
- Minimum of three years of hospice nursing experience with management experience preferred
- Skilled in nursing practice able to cope with family emotional stress and tolerant of individual lifestyles.
- Demonstrated ability to work independently with minimal supervision.
- Knowledge of Medicare Medicaid private insurance and other community programs and resources.
- Demonstrated ability to work effectively on an inter-disciplinary team.
- Excellent customer service philosophy and skills required.
- Demonstrated organizational and time management skills.
- Basic computer skills to include but not limited to Microsoft Word and Outlook required.
- Good oral and written communication skills.
- Must have reliable transportation.
