This role is part of the Check-In Community Mental Health team based at Frontyard Youth Services in Melbournes CBD. Frontyard Youth Services is located in Melbournes CBD as part of Melbourne City Mission (MCM). The mission is to support young people aged 12 to 25 in meeting their physical emotional and social needs while also guiding them towards pathways out of homelessness. Frontyard offers a dedicated youth homelessness access point crisis accommodation with 18 beds wraparound support services ranging from physical health to community connection plus outreach programs spanning Greater Melbourne and Victoria.

The Frontyard Check-In Program was developed in 2017 and has recently been enhanced to respond to the recommendations of the Royal Commission into Victorias Mental Health System. This enhanced program aims to ensure no one falls through the gaps of mental health service provision.

The program aims to:

  • Provide early and crisis intervention mental health support to young people aged 12-25 years presenting with complex mental health issues.

  • Achieve short- and long-term mental health and social goals with an aim to improve overall health wellbeing daily living skills and social connectedness.

  • Support better integration and transition to acute and non-acute mental health services.

  • Develop and maintain partnershipswith service providersto facilitate referrals and access to appropriate care in a timely manner.

About the opportunity:

This role sits outside of a clinical setting and within a youth work environment where youll be working hands-on with young people aged 17-25 at Frontyard. This role emphasises engagement relationship-building and practical skill development to support young people in achieving independence and working towards psychosocial recovery. If you are looking for variety and connection in your work this space provides an opportunity for no two days to be the same with relational practice driving many of our outcomes.

The primary responsibility of this role is to provide mental health OT expertise to support young people with complex mental health and social concerns who are part of the Check-In Program at Frontyard Youth Services. Additionally the role will provide leadership support to the Grade 2 Mental Health OT and contribute to the leadership of the Check-In program team. You will contribute to developing and conducting an efficient effective and therapeutically focused OT service along with managing a small caseload of young people.

In this unique role you will focus on informal assessments and practical interventions rather than formal clinical assessments. As part of the Check-In Team at Frontyard youll work in a dynamic youth work environment and play a pivotal role in delivering efficient effective and hands-on therapeutic services with plenty of opportunities to connect and engage in fun activities with young people.

Interventions may encompass a wide range of areas such as function and capacity sensory modulation social skills vocational activities and addressing physical health or substance use concerns. Youll engage with clients on a 1:1 basis or within small groups conducting holistic health assessments facilitating client-driven goal setting and developing care plans that empower clients to take charge of their own well-being.

Classifications for this position will range from Occupational Therapist Gr 3:
VG3 - VG6 ($104135.20 - $118981.20) pro rata.

About you:

We are looking for someone who:

  • Has Occupational Therapy qualifications and current AHPRA registration.

  • Is eligible for membership with Occupational Therapy Australia Association.

  • Brings a minimum of 7 years experience in relevant acute community and mental health settings.

  • Demonstrates a commitment to trauma-informed strengths-based evidence-based and recovery-oriented approaches.

  • Excels in interpersonal communication problem-solving and client-centred care.

This position is part of the HealthCare for the Homeless Program (HfH). HfH comprises several services that support clients that are experiencing or at risk of homelessness. DUTIES

  • Work within a holistic traumainformed and Strengths Model of recovery orientated practice.
  • Deliver targeted practical interventions with clients to support their short- and long-term goals towards achieving psychosocial recovery individual independence and identified vocational activities of interest.
  • Build a therapeutic alliance with identified clients and actively engage them in the planning delivery and evaluation of their care.
  • Deliver specialist programs to individuals or to groups.
  • Manage a case load of clients and provide clinical leadership to Grade 2 OT.
  • Undertake holistic comprehensive assessments in collaboration with the client using motivational interviewing and self-management approaches.
  • Collaborative care planning with clients carers and/or families in the context of their health functional psychosocial cultural environmental and economic needs
  • Support the development of enhanced care and treatment plans that complement existing plans and promote improved recovery and outcomes and
  • Utilise discipline-specific OT skills to deliver a range of evidence-based assessment and interventions as required by clients and in line with current best practice.
  • Support client generated goals that are realistic time limited and based on addressing health and wellbeing needs.
  • Identify and access multidisciplinary and specialist supports to address specific client issues.
  • Assessment and management of risk issues including timely documentation of the plan and escalation.
  • Monitor health outcomes and plan for the transition and exit from the program ensuring appropriate long term community supports are in place on discharge.
  • Provide supervision and clinical support to junior staff.
  • Ensure all documentation clinical handover and observations are completed each shift and in line with required SVHM and program standards

CRITERIA

6.1ESSENTIAL REGISTRATION LICENSE OR QUALIFICATION REQUIREMENTS

  • Occupational Therapy qualifications

  • Sound working knowledge of and a commitment to the recovery approach in mental health with a focus on working collaboratively with clients to promote self-determination and enable progress towards reaching personal goals.
  • A comprehensive understanding and working knowledge of the Mental Health Act 2014 (including amendments) and other relevant legislation.
  • Commitment to working collaboratively and effectively with multiple key stakeholders both internal and external of St. Vincents Health (including clients carers family members and community agencies).
  • Highly developed interpersonal and communication skills including problem solving ability and conflict resolution skills as well as sound oral and writing skills.
