Greenfield Rehabilitation Agency is excited to welcome a dedicated and passionate physical therapist to our team at our newly acquired senior community in Markesan WI. This Full time Part-time and PRN opportunity is ideal for a physical therapist who values flexible scheduling clinical excellence functional evidence based practice and the support of a therapist-owned organization.
As a physical therapist with Greenfield youll work closely with residents to help restore mobility build strength and reduce pain. Youll be part of a collaborative care team committed to improving quality of life through skilled individualized treatment.

Physical therapist responsibilities:
Conduct evaluations and develop treatment plans for residents with mobility strength balance and functional limitations
Provide therapeutic interventions that promote safety independence and recovery
Educate residents and caregivers on proper movement techniques and fall prevention
Collaborate with a multidisciplinary team to ensure well-rounded person-centered care
Monitor and document progress in accordance with regulations and company standards
Contribute to care planning discussions with insight and clinical expertise
Help residents achieve the highest possible level of physical function

Qualifications
- Graduate of an accredited school with a bachelors masters or doctoral degree in Physical Therapy.
- License as required by the State of Wisconsin to practice as a Physical Therapist.
