Essential Functions:
- Coordinates inpatient and post-discharge care scheduling and reporting.
- Provides disposition planning and maintains disposition data.
- Coordinates client care with other service providers and collaborates with interdisciplinary team to ensure continuity of care.
- Coordinates available NCH and community resources to enhance client care.
- Provides education and support to patients and their families regarding treatment options and resources.
- Completes documentation of discharge and care coordination activities in Epic and other related administrative paperwork in accordance with policy standards.
Education Requirement:
Bachelors Degree in social work counseling psychology or a related field required.
Licensure Requirement:
LSW required
Skills:
- Excellent communication and interpersonal skills in order to establish and maintain positive relationships with clients families peers and community representatives.
- Ability to work with the hospitals electronic medical records system.
- Ability to work and problem-solve with minimal supervision.
- Ability to adapt to change positively in workflow and process.
- Spreadsheet data entry and tracking and computerized scheduling skills.
Experience:
Experience providing and managing behavioral healthcare to children adolescent and families required.
