Greenfield Rehabilitation Agency is excited to welcome a talented and experienced Occupational Therapist (OTR) to lead our therapy team as the Rehab Director at our highly respected senior community in Seymour WI. OTR rehab director responsibilities:
Lead and manage the therapy department ensuring quality outcomes and team alignment
Supervise and support occupational therapists COTAs and other rehab team members
Oversee scheduling staffing and daily operations to ensure smooth delivery of care
Conduct assessments and develop evidence-based treatment plans to improve residents function and independence
Collaborate with the interdisciplinary team to meet the holistic needs of each resident
Monitor progress adjust treatment plans as needed and ensure regulatory compliance
Foster a positive and productive work culture built on teamwork accountability and respect
Serve as a liaison with facility leadership and ensure integration of rehab into the broader care model
Maintain documentation standards and participate in audits and quality improvement initiatives
What were looking for in an OTR rehab director:
Licensed occupational therapist in the state of Wisconsin
Leadership experience in a skilled nursing or senior living setting preferred
Excellent interpersonal clinical and organizational skills
Passion for empowering older adults and leading high-performing teams
Ability to balance hands-on clinical work with administrative responsibilities
Commitment to resident-centered care and professional development

Qualifications
- Graduate of an accredited school with a bachelors masters or doctoral degree in Occupational Therapy.
- License as required by the State of Wisconsin to practice as an Occupational Therapist.
