Compensation & Differentials

The starting rate for Direct Support Professionals (DSPs) is $18.36 per hour. HeartShare offers opportunities to increase your hourly rate up to $20.36 per hour through the following differentials:

  • Driving Differential:Candidates with a valid drivers license who are assigned to drive for HeartShare and successfully complete HeartShares driver training and assessment upon hire will receive an additional pay differential.
  • AMAP Differential:Candidates who currently hold an AMAP (Approved Medication Administration Personnel) certification or who successfully complete HeartShares AMAP training and assessment upon hire are eligible for an additional pay differential.

Note:You do not need to have these certifications prior to hire HeartShare provides all required training and assessments. These differentials offer great opportunities to grow your skills and increase your earnings.

Position Summary:

The Day Hab Support Professional is responsible for implementing person-centered services for each program participant. This includes ensuring implementation of the individuals goals/valued outcomes in relation to both center-based and community inclusion activities. In addition the Day Hab Support Professional is directly responsible for the individual program participants overall safety and well-being.

Primary Responsibilities and Essential Functions of Position:

Under the supervision of the Group Leader the Day Hab Support Professional will:

  • Implement individual program participants Day Services Plan consistent with their specific goals and valued outcomes. Participate in the planning and implementation of activities both at the program center and in the community that address these goals and outcomes.

  • Train/teach program participants according to their individual goals and valued outcomes. Facilitate skills development socialization and active participation in conjunction with group room and community activities. Ensure that support is provided with consideration for each persons individualized needs and routines.

  • Facilitate inclusion of program participants in their community according to their individual needs interests and desires. Support program participants in forming and maintaining friendships with neighbors co-workers and peers. Encourage interaction between individual program participants and community members. Remain focused on facilitating the development of personal relationships for individual program participants.

  • Transport program participants in agency vehicles as required for community inclusion recreational purposes and to and from their home.

  • Participate in planning group recreational trips and outings.

  • Participate as requested in the Interdisciplinary Team meetings for program participants for whom direct support and services are provided.

  • Assist in timely collection of consumer-related information including behavioral data goal-related data community inclusion logs communication books etc. ensuring their accuracy and completeness.

  • Follow all agency state and federal regulations to maintain program compliance. Complete all written reports and documentation on a timely basis.

  • Ensure that program participants are clean and safe at all times. Ensure that no program participant is inappropriately left alone or unsupervised.

  • Participate in bussing lunchtime meals and toileting of program participants as assigned.

  • Accompany program participants to clinic and/or emergency room visits as assigned.

  • Provide crisis intervention and counseling as required.

  • Maintain confidentiality of program participants at all times.

  • Report all incidents including allegations of abuse and neglect and serious reportable incidents to program management immediately upon discovery.

  • Ensure that group room equipment and supplies are appropriately and safely stored.

  • Assist in ensuring overall program orderliness and cleanliness particularly in assigned group room. Report any hazards or unsafe conditions immediately to the supervisor.

  • Use positive approaches when handling difficult situations. Avoid gossip and negative discussions. Be flexible and adapt to change.

  • Advocate appropriately and professionally on behalf of program participants as required.

  • Serve as a role model for program participants. Abide by agency dress code. Always conduct oneself in a professional manner.

  • Remain sensitive and responsive to cultural differences between program participants and program staff.

  • Participate in in-service training and on-site workshops as required for professional growth and development. Attending external training and workshops in the direction of direct supervisor or program management. Maintain certification in CPR/First Aid and SCIP as required.

  • Represent HeartShare both within and outside the organization in a manner that promotes the mission vision and values of the agency. Personally Adhere to HeartShares Mission Vision and Values Statement in addition to all other agency policies and procedures.

  • Undertake any additional tasks and initiatives as assigned by supervisor or program management.

Qualifications

  • High school diploma or GED required.
  • CNA (Certified Nursing Assistant certification) or possess a Direct Support Professional (DSP) certificate from an accredited public or private organization preferred.
  • One or more years experience working with individuals with developmental disabilities preferred.
  • Valid NYC Drivers License Required.
