As an LPN with Traditional Home Health and Hospice you are able to give each individual patient your undivided attention as you care for them.
Job Title: Licensed Practical Nurse (LPN) - Hospice
Shift: Full-time part-time and per diem schedules available. Our office hours are Monday thru Friday 8:30am to 5:00pm.
Job Location: Wilkes-Barre PA and surrounding areas.
Our nurses see patients in the most ideal setting - where the patient resides!
At Traditional we do our best to keep each LPNs case load within a sensible geographic territory.
Job responsibilities include but not limited to the following:
- Assesses patient status administers patient care personal care and teaching of the patient in accordance with physician orders.
- Reports patient status changes care and progress to physician and nursing supervisor on a continuous basis.
- Timely and accurately prepares maintains and submits meticulous documentation as required by Traditional Home Health Care including all interactions with patients case conferences medication changes and various OASIS time points.
- Informs patients of other services that are available and makes referrals to other community resources as needed.
- Current license as a Licensed Practical Nurse (LPN) in Pennsylvania
- Must have current drivers license
- Ability to travel to case assignments
- Excellent assessment skills documentation skills and ability to meet deadlines
- Ability to work independently with minimal supervision and manage time effectively
- Strong interpersonal skills.
- Demonstrates good verbal and written communication and organizational skills
- Acute care and/or home health experience is preferred.
- Having experience with hospice patients or in the hospice environment is a plus.
- Understanding of Hospice philosophy and principles of death and dying.
