As part of a vibrant team delivers aid verbal cueing reminding and direct hands-on care for the elderly convalescents or persons with disabilities within the scope of practice to SummitWest Care clients. Essential Functions 1. Provides nursing care to client consistent with physician orders established care plan client choice and regulations. 2. Accountable for observation of the client and documentation regarding observations/care given. 3. Updates client s field chart and medication sheets when changes occur as a result of communications with other members of the team and/or care provided. 4. Coordinates client s care with physician family members of the interdisciplinary team and other involved agencies. 5. Updates client/caregivers/supervising RN on progress equipment needs and health needs and obtains and processes physician orders when appropriate. 6. Provides teaching and instruction to clients /caregivers to promote physical and emotional health. 7. Maintains clients self-esteem privacy and confidentiality of personal information. 8. Assures client rights are protected and that all clients/caregivers are made aware of the client s rights and responsibilities. 9. Performs other duties within scope of practice as assigned by RN supervisor or Clinical Manager. Maintains clients self-esteem privacy and confidentiality of personal information. 10. Initiates emergency procedures as appropriate. 11. Clinical and administrative documentation adheres to agency professional and regulatory guidelines for timeliness and content. 12. Monitoring of telehealth program 13. Communication/education to patients on disease process. 14. Timely communication with physicians regarding vital signs out of set parameters 15. Other duties as assigned. Utilizes established lines of authority and communications: 1. Communicate clear concise relevant non-judgmental information. 2. Maintain open lines of communication with all staff. 3. Utilizes effective problem-solving/conflict resolution skills. 4. Promotes supportive group process. Demonstrates personal and professional accountability: 1. Complies within accepted ethical conduct and professional standards of The Nursing Practice Act as set forth by the American Nurses Association and by the State Board of Nursing in the State of Colorado. 2. Required Education and Experience 1. Graduate of an approved School of Nursing 2. Currently licensed and in good standing in the State of Colorado 3. Previous home care experience preferred. 4. Valid Drivers License and Active Auto Insurance Work Environment The following work environment characteristics represent those an employee typically encounters while performing the essential functions of this position. This job operates 60% of the time indoors; 40% of the time outdoors. Occasional exposure to physically and/or verbally aggressive clients. Noise level is generally moderate. Job tasks involve exposure to blood body fluids and/or tissue. Physical Demands The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job the employee will stand 50% of the time sit 10% of the time and walk 40% of the time. The employee frequently is required to climb balance stoop kneel crouch crawl and reach with arms. The employee must be able to grasp pinch and feel with hands and fingers and be able to lift carry push pull or otherwise move objects up to 50 lbs. frequently and up to 100 lbs. occasionally. Specific vision abilities required by this job include close vision distance vision color vision peripheral vision depth perception and ability to adjust focus. Specific hearing abilities required by this job include ability to perceive the nature of sounds at normal speaking levels with or without correction. Must be able to receive detailed information through oral communication and to make the discriminations in sound.
