Job Description

Position: Caregiver
Work Location: Phoenix AZ 85014
Pay/Salary: $15.50/Hourly
Duration: 3 months contract with high possibility of extension or conversion

Shift: Monday - Wednesday one week and on the alternate week will be Monday - Thursday from 7p.m.-7a.m.
Dress Code: scrubs

Job Description:
Member Care Personal Caregivers provide a variety of non-medical services that assist Clients with their activities of daily living. Member Care Personal Caregivers will be responsible for providing both companionship and hands on non-medical services to ensure the safety and well-being of a client.

Essential Duties and Responsibilities:

  • Follow care plan as directed by supervising nurse
  • Complete all portions of Daily Progress notes
  • Ensure the health safety and wellbeing of the clients
  • Model appropriate social behavior and effective time management
  • Ensure clients comply with house rules
  • Engage with clients to follow their treatment plans which include personalized goals appropriate interpersonal relationships skill building and recreational activity in the home
  • Follow program guidelines and protocols and implement day-to-day structure which may include redirection and verbal/physical crisis intervention if needed
  • Assist clients with Activities of Daily Living by following their treatment plan providing Personal Care (Caregiver) services as directed
  • Assist clients with Independent Living Skill tasks including laundry cleaning rooms meal prep etc.
  • Complete Direct Care daily duties including cooking cleaning securing food/chemical cabinets etc. which are posted at the house
  • Complete staff assistance for the self-administration of medications for clients and document as directed
  • Complete the shift change process with staff which you are replacing at the start of your shift and which replace you at the end of your shift
  • Other duties as assigned

Work Environment:

The work environment characteristics described here are representative of those an employee encounter while performing the essential functions of this job. The employee will normally work in a hospital/clinic environment with frequent exposure to electronic and medical office equipment.

Physical Demands

  • The employee is frequently required to sit; climb or balance; and stoop kneel crouch or crawl. Prolonged periods of standing bending and lifting. The employee must lift and/or move up to 50 pounds.
  • The employee will engage in movements that involve lifting equipment used in patient care etc.
  • Specific vision abilities required by this job include close vision distance vision colour vision peripheral vision depth perception and ability to adjust focus. The employee may be exposed to infectious diseases and human waste products.

Required Education and Experience :

  • High school diploma or equivalent required. Must possess a strong service orientation including active listening critical thinking skills and close monitoring of clients. Basic understanding of medical terminology.
  • Minimum of two years of previous experience as a Caregiver preferred. Those working as a certified home health must complete formal training and pass a standardized test.
