Job Description: Behavioral Health Hospital Liaison
Location: Warren MI 48093
Job Summary:
We are seeking a dedicated Behavioral Health Hospital Liaison to join our team in Warren MI. The ideal candidate will be responsible for establishing and maintaining relationships with hospitals to facilitate the referral process for patients in need of behavioral health services.
Key Responsibilities:
- Collaborate with hospital staff to identify patients who may benefit from behavioral health services
- Coordinate the referral process for patients transitioning from the hospital to behavioral health facilities
- Provide education and support to hospital staff on behavioral health resources and services
- Ensure timely and accurate communication between hospitals and behavioral health facilities
- Monitor and track patient outcomes to ensure continuity of care
Skills and Qualifications:
- Previous experience as a Behavioral Health Hospital Liaison or similar role
- Strong communication and interpersonal skills
- Knowledge of behavioral health services and resources
- Ability to work collaboratively with hospital staff and behavioral health professionals
- Excellent organizational and time management skills
