General Purpose of the Quality Care Consultant Position:
The Quality Care Consultant works collaboratively with the Director Value Based Care Quality staff and practice staff to fulfill the role of lead quality representative at the practice location. Assists with identification of high risk chronic disease and high-cost patients in need of assistance in coordination of care. Provides patient education and self-management support.
Schedule: Position works Monday - Friday daylight hours and additional time as needed to complete work.
Location: Position will be scheduled to report to PPCP Central Office
Quality Care Consultant Essential Duties and Responsibilities:
- Analyzes internal and external quality reports to identify opportunities for improvement and assist with identification of select patient population in need of coordination of care or outreach.
- Actively supports and institutes within the practice any quality program initiatives and processes as outlined by the Quality Committee.
- Educates practice staff in quality initiatives.
- Collaborates with Director Value Based Care Quality staff office staff and providers to improve patient adherence to evidence based guidelines implement programs aligned with guidelines and develop strategies to overcome barriers limiting success in achieving best practice parameters. Educates patients and families about managing their health conditions.
- Provides educational materials and information on available community resources to patients.
- Collaborates with patients and families to establish and work toward appropriate self-management goals.
- Work with external community or support agencies in procuring resources for patient populations requiring additional help in achieving positive outcomes.
- Utilize reporting tools such as Enli Central Worklist to track manage and assist patient population in being compliant in partnering with practice to manage their disease states.
- Participate in development of processes to improve quality outcomes.
- Fulfill the role of lead quality representative at the practice site by educating staff and supporting quality initiatives as determined by the Quality Committee and staff.
- Identify and assist patients in need of coordination with managing their chronic disease transition of care ED usage and Medication Adherence.
- Work with office staff to help identify patient population requiring more in-depth management.
- Participate in monthly Quality Care Meeting to standardize quality care management / care coordination in the organization.
- Educate patients and families on managing their health conditions.
- Identify and address barriers to achieving positive outcomes.
- Educate and use non-physician staff to manage patient care.
- Track patient outcomes for revisions in strategies.
- Utilize evidence-based guidelines to help promote office staff commitment and understanding of care directives.
- Utilize standing orders for ordering medication refills ordering tests and routine preventive services and arranging follow-up appointments etc.
- Assist with patients navigation through the health care continuum.
- Work collaboratively with all Quality staff to optimize the clinical management of select patient population.
Qualifications:
Education: High School Diploma or GED is required.
Experience/On the Job Training:
- Minimum3-5 years clinical experience
- Minimum1-year Quality or Case Management experience
- Excellent health assessment and communication skills
- Independent and creative thinker
- Ability to problem solve
- Support patient and family problem solving self-management goals setting etc.
- Able to work well with Office staff Quality staff and providers to problem solve and implement changes
- Utilize data tracking and trending reports to revise goal setting strategies etc.
Credentials: RN LPN or Certified Medical Assistant preferred.
