Position Summary

Are you interested in being a positive agent of change Helping people with what they need when they need it The Trinity Alliance Social Health Manager will support comprehensive compassionate service to Capital Region community members. This position leads a team of community health workers and a housing resource navigator who address a range of social health challenges (e.g. health insurance navigation eviction prevention transportation coordination ID/documentation assistance etc.).This position will be responsible for coordinating health literacy-related programs and events in collaboration with community partners as well as generating and assigning grassroots outreach opportunities to engage our community in services.

Key Responsibilities:

  • Incorporate best practices regarding confidentiality into all job duties and communications in accordance with HIPAA Trinity policies and procedures and other applicable regulations.
  • Lead a team with a range of talents and responsibilities providing regular supervision coaching mentoring and support.
  • Oversee grants and contracts pertaining to our health and wellness programs; ensure fidelity is maintained throughout program implementation.
  • Implement relevant training tracking systems and other processes necessary to support efficient compassionate and effective service delivery.
  • Conduct quality assurance and continuous quality improvement for program outputs.
  • Develop and maintain positive relationships with community stakeholders.
  • Develop grassroots and innovative outreach opportunities to enhance accessibility of services.
  • Develop and implement relevant programs/events that are responsive to the community.
  • Plan and conduct program evaluation.
  • Produce required and requested reports in a timely and accurate manner.
  • Support with grant writing as needed.
  • Work as part of the department and agency leadership teams to identify opportunities to strengthen and enhance service delivery and perform any additional duties as assigned.

Qualifications:

  • Licensed social health professional (LCSW LCSW-R LMHC) or masters degree in a social or public health-related field.
  • Two or more years of experience supervising individuals providing direct services in the community.
  • Excellent crisis intervention case management advocacy skills; attention to detail and refined organizational skills.
  • Experience with data entry and management.
  • Familiarity with community-based organizations and providers of services and programs.
  • Valid drivers license insurance and good driving record.

Salary: $60000 - $65000

Full-time - 35 hours per week
