Location: Visits across Orange County Los Angeles and Inland Empire

Schedule: Full-Time

Compensation: $95000- $115000 annually commensurate with experience

Position Summary:
We are seeking a compassionate and skilled Registered Nurse to join our Palliative Care Program. This role focuses on providing holistic patient-centered care in various settings including patients homes assisted living facilities and skilled nursing facilities. You will play a vital role in delivering high-quality palliative care supporting patients and their families during challenging times.

Key Responsibilities:

  • Creatively manage resources and workflows to triage patient needs and address clinical demands.
  • Coordinate with the interdisciplinary palliative care team and referring providers to implement effective care plans.
  • Facilitate connections with complementary therapies (e.g. PT/OT massage music therapy).
  • Assist with non-pharmacological interventions to alleviate symptoms and enhance psychosocial coping (e.g. meditation Reiki journaling).
  • Support patients and families in legacy work to aid end-of-life coping.
  • Ensure continuity of care and consistent team presence across rotations.
  • Engage in advanced care planning discussions and assist with completing advance directives.
  • Deliver nursing care in patient residences and document assessments within the RN scope of practice.
  • Act as a resource for patient care units outpatient clinics and staff regarding palliative care.
  • Provide education to staff patients and families on symptom management coping strategies and end-of-life care.

Qualifications:

  • Active and unencumbered RN license in California.
  • Minimum of 2 years of nursing experience.
  • Palliative Care experience a PLUS
  • Associates or Bachelors degree in Nursing.
  • Community-based nursing experience including home visits preferred.
