Job Description

JOB SUMMARY

As part of Siloam Missions vision of a Winnipeg without chronic homelessness this position involves providing case management services to residents with a focus on empowering them to make decisions set and achieve goals and maximize their self-determination. The responsibilities include assessing residents skills strengths and development needs acting as an advocate for resident needs maintaining resident records monitoring and communicating resident progress providing tenant and eviction prevention services helping residents identify vocational interests attending meetings and training and performing other duties as assigned by the Supervisor.

This position is responsible for coordinating and delivering support services to the residents by assisting them to achieve their goals and reach personal potential.

KEY DUTIES & RESPONSIBILITIES

  • Provide case management services to residents helping them to maximize their ability for self-determination by supporting and empowering them in decision-making and assisting with goal setting planning and follow through of identified goals.
  • Assess resident for relevant strengths and areas requiring support.
  • Act as an advocate for the needs of the residents of the Madison.
  • Maintain resident records in a timely manner.
  • Monitor evaluate and communicate progress of residents with the resident themselves team members and supervisor on a regular basis.
  • Support residents with eviction prevention services as needed.
  • Assist residents in determining their strengths and interests and pursuing vocational related activities.
  • Attend bi-weekly team meetings attend staff meetings as required.
  • Participate in all mandatory training.
  • Other duties relevant to the position as assigned by Supervisor.

EDUCATION & EXPERIENCE

  • Completion of a health or human service degree/diploma in related fields. Combination of education and experience may be considered.
  • Experience in case management in the social services field with focus on assessment service planning evaluation follow up review and closure.
  • Understanding of systems impacting Madison residents (such as Employment and Income Assistance home care healthcare etc.).
  • Experience working with persons with mental health illness and/or addiction concerns.
  • Certification in ASIST Mental Health First Aid NVCI and First Aid are preferred but can be trained on site.
  • Must have valid Class 5F drivers license an acceptable driver abstract access to a vehicle in good condition and be willing to drive residents to appointments.

KNOWLEDGE/SKILLS/BEHAVIORS

  • Excellent person-centered social service planning skills with a proven ability to engage diverse community members in collaborative planning efforts.
  • Extensive experience working with individuals from low-income backgrounds who also may experience challenges related to addiction mental health physical health and developmental disabilities.
  • Demonstrated exceptional skills in conflict resolution.
  • Good understanding of colonization and its impact on Indigenous peoples.
  • Experience with HIFIS is an asset.
  • Experienced user of Microsoft Office.
  • Excellent interpersonal skills.
  • Excellent written and verbal communication skills.
  • Ability to practice good organizational skills.
  • A compassionate and empathetic outlook and the ability to convey these characteristics in relationship with persons who have experienced marginalization.

RELATIONSHIP

  • Interact daily with residents and prospective residents.
  • Interact daily with interdepartmental staff and management.
  • Liaise advocate and collaborate with agencies throughout the sector.

WORKING CONDITION

  • Hours of work will be weekdays 8:00 am- 4:30 pm with occasional weekend and evening hours when determined by need and 2 overnight trips per year
  • Part of the on-call rotation.
  • Office environment within a residential facility.
  • Hazardous conditions could potentially include unpredictable or risky behaviours from community members.
  • Must have valid Class 5F drivers license an acceptable driver abstract access to a vehicle in good condition and be willing to drive residents to appointments.

WORKPLACE HEALTH AND SAFETY

The incumbent contributes to making the organization safe for residents and staff and recognizes the importance of reporting unsafe situations and participating in follow up reviews as a learning opportunity.

  • Provides a safe environment by ensuring adherences to Workplace Safety and Health
  • Regulations and Policies Infection Control Guidelines WHMIS and Safe Work Procedures. Immediately investigates and recommends corrective action on any unsafe acts work conditions incidents near misses injuries or illnesses.
  • Demonstrates understanding of role and responsibilities in fire prevention and disaster preparedness and participates in safety and health training programs including the facilitys Fire Disaster and Evacuation Plan.
  • Supports an environment which avoids prevents and corrects all activities or actions which may result in an adverse outcome in the delivery of services. Community Member safety is a standing item for all individual and departmental meetings.
