You will be updated with latest job alerts via emailWant to join our Team We are currently looking for a compassionate and dedicated LPN to join our Anne Arundel County Field Team. Our LPNs under the direction of the registered nurse provide care to our patients in all settings including patient homes and facilities death visits continuous care and GIP.
This position is Fulltime; Monday Friday 9am 5:30pm.
Coverage Area: Anne Arundel County Patient homes and facilities.
Main Responsibilities:
Recognizes changes in clinical status. Requests assistance from supervisor or an RN as needed.
Serves as a preceptor to students/new employees. Identifies learning needs of students and shares with RN to provide for optimal clinical practice.
Obtains and documents a complete list of the patients current medications and compares the medications that the organization provides to those on the patients list. Ensures that the information is available on to all care providers.
Collects data regarding family needs for bereavement services following the patients death and conveys information to bereavement staff no later than three days after death of patient in conjunction with the primary nurse.
Performsnursing procedures according to nursing policy/procedures practicing within the scope of practice.
Consistently utilizes effective time management and performs functions efficiently.
Actively participates under the direction of the RN in the initiation/revision of the plans of care and/or the initiation/revision of discharge plan within established timeframes as per documentation.
Under the direction of the RN establishes measurable patient goals and communicates ongoing plan of care appropriately to multidisciplinary care team.
