Overview:
We are seeking a Family Nurse Practitioner (FNP) to join a wellestablished and patientcentered primary care practice in Oxford Connecticut. Key Responsibilities:
Provide comprehensive primary care services for patients of all ages
Conduct wellness visits chronic disease management and acute care visits
Develop and implement personalized treatment plans based on evidencebased guidelines
Collaborate with physicians nurses and support staff to ensure highquality care
Educate patients and families on preventive care and lifestyle modifications
Maintain accurate and timely documentation in the electronic health record (EHR)
Requirements:
Graduate of an accredited Family Nurse Practitioner (FNP) program
Active or eligible Connecticut APRN license
Board Certified or Board Eligible as a Family Nurse Practitioner
Strong clinical skills and commitment to patientcentered care
Excellent communication and interpersonal abilities
Previous experience in primary care or family medicine preferred but not required
