JOB REQUIREMENTS
Minimum Education
Graduate of a Program in Discipline Required
Bachelors Degree preferred.
Required Skills
Must be able to work independently
Addresses wound care consults timely
Ability to teach staff on wound care staging & documentation
Professionally interact with patients
Ability to teach patients how to manage their wound care
Facilitates Pressure Injury monthly meeting
Accurately assesses patient skin integrity
Prevent skin breakdown in hospitalized patients
Accurately stage and assess wounds
Microsoft office skills Word Excel PowerPoint
Certifications:
Basic Life Support (BLS) to be obtained within 6 months
One to two years related experience and/or training preferred.
Required Licenses
West Virginia United States Other
Current licensure by WV State Board of Nursing.
Minimum Work Experience
Wound Care Certification preferred.
Two years of experience in wound ostomy and continence care required per Wound Ostomy Continence Nursing Certification Board.

JOB SUMMARY
Coordinates clinical activities of the Inpatient Wound Care Program which include but arent limited to utilization of skills in organization planning implementing evaluating and providing care through use of hospital and nursing standards.
ESSENTIAL FUNCTIONS
Performs assessment and plans patient care.
Evaluates the patient assessment planning and intervention process.
Demonstrates professional and personal development.
Delivers direct patient care appropriate to the age of patient served.
Assess and diagnose wounds ostomies and continence disorders
Develop and implement treatment plans
Monitor and evaluate patient progress
Provide patient education on selfcare and disease management
Collaborate with other healthcare professionals to ensure holistic care
Stay uptodate with the latest research and technology in wound ostomy and continence care
Document patient progress and treatment plans accurately and thoroughly
Provide emotional support to patients and their families
Manage and maintain specialized equipment and supplies
Advocate for the needs and rights of patients with wounds ostomy and continence disorders
Manage complex wounds ostomies and continence issues
