Job Description
Empath Home Health a member of Empath Health is seeking a PRN Lymphedema Certified OT Occupational Therapist. The Home Health Lymphedema Certified OT Occupational Therapist provides occupational therapy services related to patients level of function and performance of daily living.
Locations: home health visits available in Manatee county
Schedule: PRN
The Home Health Lymphedema Certified OT Occupational Therapist:
- Provides treatment to the patient in compliance with agency policies and procedures and federal and state regulations.
- Performs initial and on going assessments to determine level of functioning and patients ability to perform activities of daily living including OASIS assessments at appropriate time points.
- Develops and revises a plan of care in consultation with the physician and other care team members which will enable patient to achieve realistic goals of selfcare.
Requirements:
- Bachelor Degree in Occupational Therapy.
- Florida licensure in occupational therapy.
- Minimum of one (1) year experience as a licensed Occupational Therapist.
- CLT Certified Lymphedema Therapist in the respective state.
- Home health care experience preferred.
