What Youll Do:
As a Home Health Admission RN youll be on the front lines of careproviding comprehensive Start of Care (SOC) visits building strong care plans and ensuring each patient receives the personalized highquality care they deserve from day one.
Key Responsibilities:
- Conduct and manage comprehensive inhome Start of Care (SOC) assessments
- Develop and implement individualized patient care plans in coordination with the interdisciplinary team
- Deliver safe evidencebased care to diverse patients in a home setting
- Maintain timely and accurate documentation using EMR software mobile devices and tablets
- Collaborate closely with physicians case managers and support staff to ensure seamless care delivery
- Meet charting deadlines and compliance standards
- Manage your own schedule and patient visits with professionalism and efficiency
Schedule:
- Option 1: MondayThursday 8:00 AM 6:00 PM
- Option 2: TuesdayFriday 8:00 AM 6:00 PM
- Expect to complete 3 Start of Care visits per day
What Youll Bring:
- Associate Degree in Nursing (ADN) or higher
- Active RN license in the state of Illinois
- Previous experience in home health acute care or a similar patientfacing role
- Strong organizational skills and the ability to selfmanage time and scheduling
- Comfortable using tablets mobile phones and EMR systems
- Compassionate approach with excellent communication skills
