Develop individualized therapy plans based on each patients specific needs goals and abilities. - **Therapeutic Interventions**: Provide a variety of treatments including manual therapy therapeutic exercises balance training gait training and use of modalities (heat cold electrical stimulation) to improve movement and manage pain. - **Goal Setting & Monitoring**: Set measurable and realistic short- and long-term goals for patient recovery. Continuously track patient progress and adjust treatment plans as needed. - **Patient & Family Education**: Educate patients and their families about exercise programs home safety pain management and strategies for maintaining or improving mobility and independence post-discharge. - **Collaborative Care**: Work closely with physicians nurses social workers and other healthcare providers to ensure a holistic patient-centered approach to rehabilitation and recovery. - **Documentation & Compliance**: Maintain accurate and thorough documentation of patient evaluations progress notes and treatment plans in accordance with facility policies and regulatory requirements. - **Team Support & Leadership**: Mentor and supervise physical therapy assistants (PTAs) and other team members when applicable. Foster a positive and productive work environment that prioritizes patient outcomes. - **Facility Safety & Maintenance**: Ensure that therapy equipment is maintained in safe working condition and that the therapy area remains clean organized and conducive to patient care. **Qualifications:** - **Education**: Doctorate or Masters degree in Physical Therapy from an accredited institution. - **Licensure**: Current state licensure as a Physical Therapist in good standing. CPR and First Aid certification preferred. - **Experience**: Prior experience in post-acute care rehabilitation or long-term care is highly desirable but not required. New graduates are encouraged to apply. - **Skills**: Strong clinical and diagnostic skills with the ability to create and implement individualized therapy programs.
