We pride ourselves on our stateoftheart facilities our commitment to patientcentered care and our team of highly skilled Summary: We are seeking a dedicated and experienced Physical Therapist to join our rehabilitation team. The successful candidate will be responsible for evaluating patients developing personalized treatment plans and implementing physical therapy interventions to help patients achieve their rehabilitation goals. The Physical Therapist will work closely with other healthcare professionals to provide coordinated and effective Responsibilities:Conduct thorough assessments of patients physical conditions and functional individualized treatment plans based on patient assessments and physical therapy interventions including exercises manual therapy and modalities to improve patients mobility strength and functional patients and their families on treatment plans home exercises and ways to improve overall and document patients progress adjusting treatment plans as needed to ensure optimal with interdisciplinary team members including physicians occupational therapists and nursing staff to ensure coordinated and comprehensive in team meetings case conferences and continuing education opportunities to stay current with best practices and advancements in physical accurate and timely documentation of patient care in compliance with regulatory and facility to ethical and professional standards set by the American Physical Therapy Association (APTA) and state licensing :Doctor of Physical Therapy (DPT) degree from an accredited state licensure or eligibility for licensure as a Physical Therapist in experience as a Physical Therapist preferably in a postacute or rehabilitation clinical skills in assessment treatment planning and communication and interpersonal skills to effectively interact with patients families and healthcare team to work independently and as part of a collaborative to ongoing professional development and staying current with advancements in physical certification (preferred).Physical Requirements:Ability to lift and move patients equipment and periods of standing walking and performing physical therapy dexterity and strength to perform therapeutic exercises and manual therapy :Competitive salary commensurate with benefits package including medical dental and vision time off and holiday savings plan with employer education and professional development and collaborative work to Apply: Interested candidates are invited to submit a resume and cover letter outlining their qualifications and experience to email address.
