Job Description
Coordinate Behavioral Health care within the PCP or Specialty Care Practices provide proactive follow up monitor progress of person receiving care and alert providers when additional or adjustments in treatments are indicated. Support appropriate medication management and facilitate communication with psychiatric prescribers. Offer brief evidenced based counseling and refer to community mental health treatment when applicable. Determine individualized care goals explore obstacles and support the utilization of community resources and services. Maintains billing and registry for all patients enrolled and support full symptom recovery using evidenced base traums informed and measurement based toools.
Qualifications :
Education & Training:
Currently licensed as LCPC LCSWC or PsyD in the state of Maryland
Work Orientation:
Minimum of five years of psychiatric clinical experience
minimum 2 years providing psychotherapy including: a) screening for mental health and substance use disorders b) brief structured intervention techniques c) evidenced based counseling techniques d) working with persons that have comorbid physical health disorders
Skills & Abilities:
a) effectively communicate with health care providers as a team
b) effectively engage persons into a therapeutic relationship
c) working knowledge of differential diagnoses
d) basic knowledge of common psychopharmacology
e) flexibility to work within a variety of healthcare environments and with a variety of healthcare professionals
