You will be updated with latest job alerts via emailWe are looking for a BHT Outreach Worker to join our team! This role will provide mental health and substance use disorder treatment linkage to resources and related recovery support to adults within the jail and justice system. The BHT Outreach Worker will focus on identifying assessing and treating individuals needs to connect individuals with appropriate services. Staff will conduct clinical assessments and screenings to determine the presence of mental health substance use or cooccurring disorders and develop personalized medically necessary service plans for each individual enrolled in the program. This position will provide followup care for individuals participating in medicationassisted treatment (MAT) programs. The Specialist will be responsible for outreach intake and ongoing support services and will serve as a liaison between the program and other community agencies specifically within the jail and justice population. The BHT Outreach Worker will report directly to the Project Director.
