We have an exciting opportunity to join the Fife Health & Social Care Partnership (Fife HSCP) as a Link Worker within Locality Planning in The Community Led Support Team supporting both Link Life Fife and the Well. The postholder will assist at the Wells and will carry a case caseload with Link Life Fife.
Link Life Fife is a community led support service provided by Fife Health and Social Care Partnership for anyone aged 18 and over who is referred via Primary Care for support to manage stress anxiety or feelings of being overwhelmed that are affecting their mental health or general wellbeing.

Well is Fife Health and Social Care Partnerships single point of contact a place where anyone over the age of 16 can dropin both in their community and online and find out information and receive general advice to help them stay well and independent within their local community.

role involves supporting people:
- To access community resources and supports following a referral from services within NHS Fife and Fife Council as well as from 3rd sector organisations
- To develop contacts links and relationships with others in their local community.
- To make meaningful connections access opportunities within the local community encourage selfmanagement of their mental health to promote improved quality of life and mental wellbeing.
- To access alternatives to statutory services and to divert away from these by exploring more creative and personalised solutions for the individual.
You will also be required to map community led solutions and resources for people experiencing mental and physical health conditions and support the Local Area Coordinators with presentations to referrers and local organisations.
The post holder will engage with people to provide information support and guidance and maintain a high standard of customer care and compassion in communication with people including making and managing appointments handling general telephone/email enquiries from the public and stakeholders. The postholder will also be required to set up and man stalls at various community events to promote Community Led Support.

The Person
The skills you will need:
- Ability to use Good Conversations and active listening skills to enable the individual to identify their own strengths and the things that are going well for them.
- Be able to work in partnership with people the local community agencies and any others to determine preferred and available supports and options for the people you are supporting to ensure the best possible outcomes for them.
- Ability to actively engage in community mapping activities in order to identify community resources.
- Ability to plan tasks and managing workload independently.
- Ability to provide effective administrative support services which include preparing and distributing reports and maintaining project case management system to a high standard
- Experience of delivering a customer focused service (Focus on Customers See How We Work Matters Framework).
- Experience of working alongside statutory agencies community development teams and voluntary sector organisations.
- Be able to actively challenge stigma and discrimination associated with mental health through contribution to antistigma campaigns and events.
- Ability to travel within Fife
Before confirming your appointment you will be required to obtain Protection of Vulnerable Groups (PVG) scheme membership through Disclosure Scotland and become a member of the relevant PVG scheme.
