Verland is hiring Exceptional People to provide Compassionate Care as an ATF Program Specialist for our Sewickley Campus

A Program Specialist working with individuals who have intellectual and developmental disabilities is the primary advocate for each individual assigned and will ensure that all medical concerns are addressed. The PS develops supervises and coordinates each Individual Support Plan (ISP) for the individuals assigned completes assessments identifies strengths needs likes interests and preferences as well as the dislikes for each individual. The Program Specialist This is a salaried exempt position.


Contributions:

  • Must be fluent on all assigned individuals Intermediate Care Plans (PPs).
  • Responsible for securing developing & coordinating the necessary services provided to an individual as specified by their IPP as well as implementing supervising monitoring and evaluating those services as required under Regulation 2380.188 (relating to Provider Services).
  • Develops a program schedule for the assigned individuals to best meet their needs; maximize skill development and ensure that community & recreational outings are regularly scheduled.
  • Ensures all necessary content is included in the IPP (protocol for social emotional environmental plan fire safety training etc.).
  • Evaluates assigned individuals in the affective communication cognitive social and vocational development areas to determine their strengths as well as their program needs.
  • Coordinates programming plans to include crossover goals to and from the residential setting in the skill areas of mealtime social communication and toileting.
  • Monitors individuals progress monthly through direct observation written review as well as signed and dated monthly documentation regarding each individuals participation and progress toward identified outcomes.
  • Completes and ensures all necessary documentation associated with the IPP including Quarterly Reviews annual IPP reviews and the accuracy of reports (i.e. accurate content dates and signatures) as required under Regulation 2380.181(f).
  • Provides assessments to the individuals QIDP at least thirty (30) days prior to IPP meetings as per licensing requirements.
  • Responsible for reporting content discrepancies or changes related to an individuals needs as well as recommending a revision to a service or outcome in their IPP to the QIDP and applicable IDT members as required under 2380.186(c) (4).
  • Accountable for informing IDT members of the option to decline the IPP review documentation as required under 2380.186(e).
  • Responsible for reviewing the IPP as well as any updates or changes to the IPP with the individual as required under 2380.186.
  • Responsible for documenting the review of the plan and for providing said documentation to the QIDP and applicable IDT members as required under 2380.186.
  • Adheres to the Interdisciplinary Team process in all situations that will impact the Individual Program Plan.
  • Follows up on all expressed concerns and document the issues actions and progress in a timely manner.
  • Participate in at least 24 hours of annual inservice training as required by governing regulations.
  • Provide a safe healthy and clean environment for the Individuals and work to ensure and promote the health safety and mental/emotional welfare of all individuals.
  • Assist in the orientation/training of new staff members.
  • Conducts classroom meetings and provides ongoing training to applicable staff members relevant to each individual in the following areas: IPP content and reviews specific therapeutic needs health and safety needs diet orders and specific day programming needs.
  • Understands and adheres to the obligations of Verland as dictated in the current Union Collective Bargaining Agreement (CBA).
  • Participates in their individuals morning routine helping with dressing personal care and breakfast when needed. Monitors breakfast and documents Mealtime Observations.
  • Maintains a professional attitude at all times when representing the individual and the facility and fosters excellent working relationships with all staff members and peers.
  • Accesses the minimum amount of information necessary to perform job duties and adheres to current HIPAA and Verlands Privacy and computer policies.
  • Participates in the weekend OnCall rotation once a month; and may act as a Certified Investigator on an oncall basis when necessary and certified to do so.

Requirements

Required Skills & Abilities:

  • Participation in the weekend oncall rotation once a month
  • Act as a Certified Investigator on an oncall basis when necessary; willing to obtain certification if not already certified as an investigator in the State of Pennsylvania.
  • Proficiency in the use of current Microsoft Office Suite software (Word Excel Outlook Teams etc.) as well as other various types of software programs used in Electronic Health Records (EHR) administration.
  • The ability to obtain and maintain state and federally required clearances and certifications.
  • Possession of a current and valid stateissued Drivers License and the ability to maintain a clean driving record.

Minimum Training and Experience:

  • Bachelors degree from an accredited college/university in Education or a related field
  • A minimum of two (2) years working experience in the ID/DD field; preferably with individuals who had severe and/or profound disabilities.

Physical Requirements:

  • Prolonged periods of time sitting at a desk and working on a computer.
  • The ability to constantly sit stand walk as well as the ability to see hear speak write push pull climb twist and possess manual dexterity.
  • Able to easily move around campus as needed for meetings events training and companywide needs as they arise.
  • Must be able to lift and/or carry fifty (50) pounds independently bending and stretching as needed without restrictions to achieve the proper positioning/placement of individuals.
  • The ability to drive a vansized vehicle as needed to transport individuals to medical appointments field trips and outside events as needed.
