Job Description:

As a Hospice Liaison you will:
Educate healthcare professionals and families on the benefits of hospice care
Build and maintain professional referral relationships
Facilitate seamless care transitions by working with clinical teams
Represent Elara at community events and outreach efforts
Help patients and families access care that brings comfort and dignity
Bachelors degree in Business preferred
Experience in medical sales postacute care or DME preferred
Proven sales performance and ability to develop strategic relationships
Strong communication and problemsolving skills
Selfmotivated with a teamoriented mindset
Valid drivers license dependable vehicle and current auto insurance
