Job Description

Department Overview

Reporting to the Operations Manager Specialty Pharmacy Services this position is responsible for theprovision of specialized clinical and/or MTM services to patients in a community/ambulatory setting. This position is responsible for providing refill authorization services to ambulatory clinics supervision of supportstaff and ensuring compliance with all legal regulatory and institutional directives. This pharmacist willinteract directly with prescribers and clinic staff to develop an interdisciplinary approach to patient care.Additionally the pharmacist will be responsible for the development and implementation of pharmacyprogram opportunities and will be expected to contribute to the financial viability of the program departmentand institution.

Function/Duties of Position

  • Interprets and evaluates refill authorization requests for accuracycompleteness and appropriateness. Recognizes therapeutic problems includingdrug interactions contraindications improper dosing and precautions oftherapy. Recognizes actual or potential problems and provides therapeutic recommendations for their resolution.
  • Provides medication therapy management medication reconciliation allergy review and collaborates with providers to ensure safe and appropriate reports all adverse drug reactions drug defects and medicationerrors.
  • Conducts therapeutic interchange under the collaborative drug therapy management program. May review authorization requests to identify andmake recommendations on formulary alternatives that support cost effectiveprescribing. Collaborates with clinic and support staff to resolve authorization issues.
  • Participates in committees supporting refill authorization protocols and Epic development of tools to support workflows.

  • Assists in the development maintenance and implementation of pharmacy programs to support overall services for patients

  • Assists with devising developing and operationalizing programs to ensure proper use of medications and continuous quality improvement.

  • Collaborates with business intelligence group to provide appropriate reporting of program outcomes

  • Provides necessary support for service level agreements with ambulatory clinics

Required Qualifications

  • A current Pharmacists license issued by the Oregon State Board of Pharmacy.
  • Community pharmacy practice experience and ambulatory pharmacy practice experience.
  • OHSU may require training or experience in a specialized area at time of recruitment.

Job Related KnowledgeSkills and Abilities(Competencies):

  • Knowledge of pharmaceutical care principles.
  • Disease related modification of drug therapy.
  • Project management skills and experience in contermporary medication use systems.
  • Strong interpersonal communication skills and ability to work in a team setting.
  • Systematic problem solving skills.
  • Ability to work independently with minimal supervision.
  • Successfully completes initial and continued competencies as required for maintenance of standard of practice legal regulatory compliance and OHSU policies.

Preferred Qualifications

  • Advanced Pharmacy Practice Residency BCPS OR 3 years of equivalent experience.
  • State of Oregon Preceptor License.
  • Experience in clinical and/or business program development.
  • Direct patient care and disease related modification of drug therapy medication therapy management medication reconciliation skills.
  • Project management skills quality improvement and development of the Specialty Pharmacy Program.
  • Computer skills including use of electronic EMR MSWord Outlook Excel and other applicable databases.

Additional Details

The pharmacist will experience competing demands on their time from the volume of requests and willneed to effectively multitask and prioritize demands. The pharmacist must deal with time constraints andneed to exercise clinical judgement on a daily basis. The expectation will be to provide clinic staff andpatients with excellent customer service. Additionally within the healthcare setting there can be asignificant lack of control over the work pace with frequent interruptions (work is often dictated byexternal factors) that may lead to mental fatigue or stress.
