POSITION SUMMARY In alignment with the mission and vision of Breakthrough Urban Ministries the Peer Support Specialist provides intentional peer support to 20 individuals across the city as referred through the database.
ESSENTIAL RESPONSIBILITIES/FUNCTIONS - Provide intentional peer support to 20 individuals across the city as referred through the database.
- Attend housing events as identified by All Chicago
- Conduct assessments and engage individuals in wellness planning.
- Facilitate referrals to wellness and harm reduction supports.
- Provide transportation to appointments when funding is available.
- Offer information on community resources to enhance knowledge of available supports and reduce social isolation.
- Promote tenancy skills through education and practical support.
- Assist individuals in applying for benefits.
- Maintain uptodate knowledge of peer support services and requirements in Illinois.
- Ensure timely and accurate documentation of services in HMIS the company database and tracking spreadsheets.

QUALIFICATIONS
- Certified Recovery Support Specialist (CRSS) credential required with lived experience in mental health and/or substance use recovery preferred.
- Experience providing intentional peer support within a community setting.
- Knowledge and understanding of recoveryoriented systems of care from a healingcentered perspective.
- Must have a valid drivers license

- Understanding of substance use disorders and effective approaches to care.
- Understanding of client confidentiality and adherence to the professional code of ethics for CRSSs.

- May support supervision of Peer Support Interns
- Direction received from the Peer Support Coordinator
PHYSICAL REQUIREMENTS AND WORKING CONDITIONS Work Environment While performing the duties of this job the employee will work in a multipurpose facility and/or outdoors and thus will be exposed to weather conditions prevalent at the time. The noise level in the work environment is usually moderate but can be high depending on the activities in the facility.
Physical Demands While performing the duties of this job the employee is frequently required to walk and/or sit; use hands and fingers to feel objects tools or controls reach with hands and arms; balance; stoop; regularly talk or hear. The employee must be able to bend stoop pull lift as well as have overhead extension of arms. Must be able to stand for long periods of time; be able to lift carry or move at least 50 pounds. Must adhere to proper handling and lifting techniques. Specific vision abilities required by the job include close vision distance vision peripheral vision depth perception and the ability to adjust focus.

Equipment Used The employee will generally use general office equipment including but not limited to telephone computer calculator copier overhead projector paper shredder paper cutter and filing systems.
