Now Hiring: Licensed Practical Nurse (LPN)
Location: Columbia SC
Schedule: FullTime MondayFriday No nights weekends holidays or oncall

About the Role:
As an LPN with Elite Home Care you will play a vital autonomous role by providing inhome visits for clients throughout the Midlands region. You will be responsible for:
Creating and overseeing personalized care plans for clients.
Conducting initial inhome signup visits followed by 30day and 90day recurring visits.
Ensuring compliance with Medicaid Waiver programs Community Long Term Care and Veterans Affairs standards.
Completing all documentation in Wellsky and the states EVV system.
Communicating with Case Managers and internal Operations Managers as needed.
Reviewing Client Rights Advanced Directives and Plans of Care with clients and/or responsible parties.
What Were Looking For:
Licensed LPN in South Carolina (required) or Compact State
Associates degree from an accredited college or university
Current CPR certification (required)
At least 21 years of age
Experience with seniors dementia care home care or individuals with special needs (preferred)
