Job Title: MENTAL HEALTH CARE COORDINATOR 912 (Temporary)

POSITION SUMMARY

The Mental Health Care Coordinator will ensure the provision of mental health service coordination for students and families identified within the district with potential mental health needs. This position will determine eligibility for coordination services assist school personnel and families in procuring needed treatment or other services monitor student progress coordinate delivery of appropriate services maintain applicable caseload records and prepare reports. The Mental Health Care Coordinator reports to the High School Principal or designee and does not supervise any employees.

ESSENTIAL RESPONSIBILITIES

  • Coordinate mental health services at Forest Grove High School
  • Develop care coordination plans for identified children and families
  • Effectively engage diverse families in the care coordination process
  • Provide technical assistance consultation and training to district staff regarding community resources and other mental health issues as necessary
  • Provide clinical consultation to schools as necessary
  • Provide leadership in the district crisis response team and help facilitate the districts suicide prevention/intervention efforts
  • Act as a liaison to provide coordination with systems of care i.e. community mental health partners chemical dependency and physical health care providers local acute hospitals DHS Child Welfare Juvenile Justice schools and others as needed
  • Provide professional development on relevant mental health topics at the building and district level
  • Supervise the monitoring of client progress by school personnel; visit school buildings attend school meetings; provide for additional services as new needs arise
  • Work with other county agencies and community organizations to coordinate effective mental health service provision to students
  • Provide information on and referrals to specialty services; arrange for a variety of goods and services related to mental health treatment
  • Seek out appropriate best practices and emerging research in the field of behavioral health and wellness
  • Clinically supervise mental health counseling interns toward the provision of services to students as needed
  • Demonstrate an understanding of how the work of the Care Coordinator fits within the context of the districts Behavioral Health and Wellness vision mission and guiding principles.
  • Participate in ongoing care coordination program development and evaluation
  • Model appropriate confidentiality requirements

OTHER RESPONSIBILITIES

  • Maintain consistent and predictable attendance to meet the requirements of the position.
  • Participate in various meetings for the purpose of receiving information or sharing information and expertise with students colleagues and parents/guardians.
  • Confer regularly with immediate supervisor
  • Perform other duties as assigned.

MINIMUM QUALIFICATIONS

Education and Experience Required

  • Masters degree in counseling marriage and family therapy social work or psychology
  • Holds a license from relevant licensing board (e.g. Oregon Board of Licensed Professional Counselors and Therapists Oregon Board of Psychology).
  • Minimum three years of experience delivering mental health services to children and families.

Education and Experience Preferred

  • Experience working in a wraparound model with multiple community providers.
  • Experience working within a school system with children and families.

Skills

  • Maintains integrity of confidential information relating to students staff or district patrons

Knowledge

  • Knowledge of the evaluation and treatment of children and adolescents with mental health disorders models of behaviorism community mental health services and caseload management.
  • Knowledge of Federal and State regulations especially educational laws applying to disabled children and child abuse laws; and knowledge of State and Federal regulations relating to mandated services for disabled students and the health and welfare of children and adolescents.
  • Knowledge of best practices in the field of social emotional learning including but not limited to trauma informed practices collaborative problem solving PAX etc.

Abilities

  • Ability to identify precipitating events; gather histories of mental and physical disabilities alcohol and drug use past mental health services and criminal justice contacts
  • Ability to assess family systemic social and school relationships
  • Ability to provide individual family and group therapy.
  • Ability to effectively communicate with individuals having mental health needs their families service providers and school personnel
  • Ability to evaluate mental health needs and to develop care coordination plans
  • Demonstrates command of oral and written English language
  • Ability to work collaboratively and establish and maintain effective working relationships with coworkers
  • Ability to effectively work and communicate with students parents and school personnel from diverse cultures or backgrounds in English Spanish or other languages related to the job
  • Ability to maintain accurate and complete records.
  • Demonstrate professionalism and appropriate judgment in behavior speech and dress in a neat clean and appropriate professional manner for the assignment and work setting

Licenses Certifications Bonding and/or Testing Required

  • Holds a license from relevant licensing board (e.g. Oregon Board of Licensed Professional Counselors and Therapists Oregon Board of Psychology).
