Job Description
Job Summary:
Are you a compassionate and detailoriented RN looking for a rewarding opportunity in the home care industry Our rapidly growing inhome senior care agency is seeking aRegistered Nurse (RN)to conduct inhome assessments visit prospective clients and assist with administrative tasks

Responsibilities:
- Conduct inhome RN assessments on clients in Richmond VA and surrounding areas
- Conduct potential prospective visits to assess a clients eligibility for services.
- Maintain accurate and organized documentation using ourcomputer systems(must be techsavvy).
- Monitor for changes in clients condition and report and document relevant information
- Assist with the development of an individualized plan of care for each assigned client
- Supervise and evaluate performance of staff and intervene when appropriate
- Oversee assigned care levels services and performance improvement activities
- Provide recommendations and education to clients and family members on disease process treatments medications and nutrition
- Communicate and collaborate with healthcare providers and other professionals
- Ensure cases are managed within defined productivity standards
- Participate in quality assurance activities such as client conferences caregiver and care plan reviews
Qualifications:
- Current RN license in Virginia
- A valid drivers license and a reliable vehicle arerequiredfor this position
- 3 years of professional nursing experience (Experience in home health senior care or a related field is aplus!)
- Proficiency in the use of computers internet applications and software
- Excellent organizational and timemanagement skills
- Ability to multitask in a fastpaced environment
- Ability to maintain patient confidentiality
- Can work or visit homes that have pets (both Cats & Dogs)
