About the Department
Social Worker role that is deeply rooted in helping people and their families navigate the complex emotions and decisions around chronic illness and living well in their last phase of life. This role will support patients at Blodgett Hospital and continue to support them after discharge.
Building trust and connection with patients and families to understand their fears values and hopes for the future. This will create a foundation for meaningful conversations about their care.
Facilitating open dialogue to guide patients in discussions on their prognosis chronicity and potential outcomes with their medical care. Encouraging transparency in conversations between patients families and the care team so the patient is in control of their care and journey.
Exploring what living well means to each person at this stage in their life. This could involve exploring their desires for quality of life physical comfort emotional wellbeing spiritual and autonomy in decision making.
Support for families loved ones and caregivers who may struggle with understanding the patients chronicity of illness and accepting patients wishes.
Each patient will have a unique perspective on what living well means during their journey and this role is crucial in helping them find clarity and peace in that process.

Scope of Work
The Serious Illness Care Navigator (SICN) will assess the patients individual needs and develop a plan of care to meet those needs. This role includes providing education and support to patients families staff and caregivers. Demonstrates expertise current knowledge in care management of a caseload of patients with high complexity and seeks to improve patient family and health systems /community outcomes through the application of education concepts/skills and planning for medical crisis and end
of life in a management care environment. SICNs will have visits with patients and families while hospitalized in postacute (skilled nursing facilities) and completed home visits to manage patient goals and needs for their health care.
