About the Role

As a Community Support Worker you will meet with individuals families and carers to connect and listen and where necessary undertake an assessment under the Care Act 2014. Where there is an eligible need you will devise a plan for how the person can access the support they require this may be connecting with their local community referral on to other agencies or provide service to support their unmet need. Where necessary you will identify and report potential safeguarding consider mental capacity and other highrisk situations where a persons safety or liberty may be at risk. Your team will have an office base and can work in a hybrid way. You will undertake visits to people in their own homes in a hospital setting or wherever the person feels most comfortable to meet you.

We have vacancies in the following teams:

  • Home First East / West Discharge(permanent 18.5 and 37 hours per week) Bassett Street & Coalville Leicestershire

Our Teams

Home First is a busy frontline team offering short term support to older adults and people with physical and mental health conditions. You will be required to work at pace and manage your own case. You will need to be assertive dynamic and resilient to manage this exciting and rewarding position. We have a great team who are supportive and knowledgeable to support your induction into our service.

We are passionate about developing your career in social care.

As a Grade 9 Community Support Worker you will be expected to have sufficient experience knowledge and skills to manage your workload under supervision to deliver a high quality person centred accountable statutory social care. This includes demonstrating competency undertaking Care and Support Assessments Mental Capacity Assessments and making Best Interest decisions.

As a Community Support Worker there are further opportunities to develop your career in Adult Social Care at Leicestershire County Council such as the opportunity to apply to undertake a Social Work Apprenticeship Occupational Therapy Apprenticeship and routes in leadership roles.
