Position Summary
Alfred Health is transforming approaches to healthcare delivery through our Interconnected Care Program. We aim to optimise patient care coordination improve patient access maximise resource utilisation and enhance the overall healthcare experience for both patients and employees through innovative technologies process enhancements and new care models.These new models of care will ensure that the quality of care received at home is as good as if not better than that provided in hospital settings.
Were looking for a dynamic and passionate Interconnected Care Program Manager to drive the operationalisation of interconnected care models and support transformational systemwide change. This position sits within the Alfred Health Operations team and is crucial in shaping patientcentred care pathways both within and outside the hospital setting. The role will drive the implementation of advanced virtual care solutions for remote care management and clinical monitoring remote consultancy and support the scaling of care outside hospital walls. Your work will directly enhance healthcare delivery extending the reach of quality care beyond traditional hospital settings.
