What Youll Do:


The Jail Based Behavioral Services (JBBS) program is to develop and provide high quality case management and clinical services. The Case Manager for JBBS provides direct case management clinical services and supportive services to individuals in Larimer County Jail and in the community who are experiencing behavioral health issues including mental illness and/or substance use disorders. This role is critical in bridging the gap between incarceration and communitybased care supporting treatment engagement discharge planning and successful reintegration. This position will also support The DUI Treatment by providing case management referral / stakeholder coordination and other client needs.

  • Location: Fort Collins Colorado
  • Status: Fulltime 40/hrs. week

Duties include but are not limited to:


Essential Duties:

    • Provide direct case management services including referrals coordination of care advocacy and linkage to community resources for the JBBS and DUI Programs.
    • Collaborate with jail staff probation/parole officers social workers treatment providers and family members as appropriate.
    • Support clients with reentry planning including securing housing mental health treatment substance use services medical care and benefits enrollment.
    • Assist clients in improving their wellbeing and quality of life by coaching in behavioral skills; educate consumers to make informed choices; develop clients coping skills; help clients to solve problems and negotiate tasks of developing a mindful lifestyle.
    • Facilitate groups
    • Participate in multidisciplinary team meetings case conferences and planning sessions.
    • Educate clients on life skills selfadvocacy and community resources to reduce recidivism.
    • Respond to crisis situations and provide deescalation support as needed.
    • Documents progress reports case management notes letters faxes and phone calls. Maintains all required paperwork for payee ships.
    • Maintain close and cooperative relationships with community agencies and other public agencies and private business in carrying out supportive services.
    • Maintain compliance with HIPAA jail protocols and agency policies.
    • Perform duties as assigned

Required:

  • Relevant bachelors degree (i.e. social work human services or related field)
  • Applicant must be able to have an applicable degree to be on the CAS Track. This would entail ongoing supervision and CAS Trainings.
  • 13 years minimum of directly relevant experience in the Mental Health Field Substance Use Disorder Field
  • Experience with mild to severe mental illness and substance use disorder
  • Experience in coordinating services with other agencies

Preferred:

  • CAS preferred
  • Bilingual / Bicultural

This position will include driving a company vehicle and require a valid Colorado drivers license.

Total Rewards

The typical hiring range for this position is $24.04 $28.85/hour based on relevant years of experience.
