Intervention Navigator (IN) (Fulltime 40 hours) Nonexempt


BUILD Inc. (Broader Urban Involvement and Leadership Development) is presently seeking a committed selfmotivated and innovative individual to join our team. BUILD is a nationally respected gang intervention violence prevention and youth development organization based on Chicagos West Side. The Intervention Navigator (IN) will coordinate and provide support and services to youth and their families through the process of assessing planning implementing coordinating monitoring and evaluating options and services required to meet the clients service plan needs and goals around mental physical and general wellbeing. Through advocacy communication and resource navigation the Intervention Team will promote equality and costeffective interventions and outcomes helping find services needed and monitor progress through the completion of service plans. Services include but are not limited to: connecting clients with additional critical resources such as mental health care medical care legal services victims rights and/or compensation interpretation services workforce development and financial education. Emotional support navigating the criminal justice system including police departments legal services and the courts. Services take place at BUILD in the community and other locations as needed. The (IN) reports directly to the Manager of Intervention Services (MIS)


Responsibilities are but not limited to:

  • Connect youth involved in the justice system and street activity in comprehensive services that promote healing foster lifeskills and prevent highrisk/consequence behaviors that make them vulnerable to violence and other negative outcomes
  • Provide support and consult with Intervention Coordinator to meet client needs
  • Contributes to participant related data systems for record keeping and grant reporting purposes
  • Ensure the timely flow of case management activities to meet program service plan goals
  • Utilizes BUILD Universal Referral form to document and connect families to other BUILD services as needed
  • Completes all assessments service plans case notes and any other significant program data within a timely manner
  • Maintains all forms required including case notes client service plans referral forms information releases related to health or disabling conditions health information and any other information necessary in program participants file utilizing all procedures related to maintaining confidentiality at all times
  • Meet with Intervention Coordinator weekly to review caseloads and documentation
  • In partnership with Crisis Response Specialist Community Wellness Navigator and Mental Health Therapist helps to identify positive adult/youth influences to create a stronger support network for the program participant
  • Regularly monitors client activity related to achieving personal and programmatic goals and provides revisions to service plans as needed
  • Establishes a regular schedule of home visits contacts and followups to encourage the building of relationships with participants and ensure completion of service plans
  • Utilize motivational interviewing practices to engage and retain participants through the completion of their service plans
  • Operates professionally in the field on a flexible schedule which may include late evenings and weekends
  • Represents BUILD to broaden community awareness and support interpreting the agencys programs and services at programrelated community coalition and committee meetings as needed
  • Attends departmental and agencywide staff meetings as scheduled and agencywide events as required
  • Maintains knowledge current in the field in way of participation of coursework seminars and/or workshops
  • Contribute to data reporting request collaboration with program manager and coordinator
  • Participate in agency outreach and recruitment events including evenings and weekends
  • Participate in civic engagement activities that advance the mission of BUILD Inc.
  • Perform other duties as assigned
Requirements
  • Minimum two years of experience working with innercity youth in a human service setting traumainformed care social service setting or related field
  • Bilingual English/Spanish preferred
  • Must demonstrate excellent interpersonal verbal and written communications skills
  • An automobile is required along with current insurance and a valid drivers license
  • Maintain respectful and appropriate relationships with program participants all levels of agency staff and the various publics of the agency demonstrating sensitivity to all ethnic racial and economic backgrounds
  • Must be willing to travel as needed
  • Must be passionate about youth development work
