An Assertive Community Treatment (ACT) team consists of a communitybased group of medical behavioral health and rehabilitation professionals who use a team approach to meet the needs of an individual with severe and persistent mental illness. The role of the ACTT Nurse is to work as a team member responsible for conducting assessments addressing physical health needs making appropriate referrals to community physicians providing management and administration of medication in conjunction with the psychiatrist providing a range of treatment rehabilitation and support services.

Job Duties Include:

  • Provide service coordination (case management) for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team
  • Provide treatment rehabilitation and support services with some interventions
  • Provide therapeutic services to consumer in line with best practice to promote recovery and stability
  • Conduct comprehensive assessment of psychiatric history mental status diagnosis physical and dental health use of drugs or alcohol education and employment social development and functioning activities of daily living and family structure and relationships
  • Assume primary responsibility for developing writing implementing evaluating and revising overall treatment goals and plans in collaboration with the client and the individuals treatment team

Basic Requirements:

  • Ability to perform tasks on the computer including working with Microsoft 360 Excel Word and other basic programs.
  • Valid drivers license auto insurance coverage and reliable transportation.
  • Pass criminal background check and other regulatory checks.
  • Provide proof of your education.

Education and Experience Required:

  • A masters degree in Counseling Psychology Social Work or closely related Human Services area and licensure listed below.

  • Exceptional written oral interpersonal and organizational skills.

  • High level proficiency in Microsoft Office MS Word Excel Access and PowerPoint. Or an equivalent combination of education and experience.
